not quite a game over screen but close enough, in Scarface:The World Is Yours, if u have the police on u 4 2 long then the screen turns sort of red and it says "YOU ARE FUCKED", impossible to lose them and i never survived for longer than a minute after that, even with the best cheats i could find

The first Pikmin's game over scene seemed so out of tune with the rest of the game that it was just sort of creepy, and in one of the Trauma Center games, whenever you fail a mission a small paragraph pops up and essentially says your character fell into depression and committed suicide

But yeah, that one gets my vote. Honorable mentions to the deaths in Clock Tower for the SNES where you run into an elevator, hear screams and then blood leaks out from under the elevator door....creepy shit.

I preferred the one where she is driving away and sees the giant scissors rising slowly from the back seat.
I say the one I just said because you both succeeded and yet failed at the same time.
