Helicopter crash (even Halo 2 had this, in the form of a pelican)
Terrorists have a nuke/bomb/the means to detonate halo-shaped mass murder weapons.

Your Dad is Dead/missing
If dead (as in really dead) somehow the main badguy had something to do with it,

If alive Evil/Went away to fight evil/captured/just an ass

Also Damn you know I want a game where the badguy has a soul and feeling, where you can actually sympathize with him/her, and it shows the good that the badguy accomplishes

i got about to number 50 and then looked at how long it was, i was just like a screw it
most of them were true apart from

29# Indestructible Weapon Rule
No matter how many times you use that sword to strike armored targets or fire that gun on full auto mode it will never break, jam or need any form of maintenance unless it is critical to the story that the weapon breaks, jams or needs maintenance.

This was true in a lot of old games, not as much so by todays standards, it really irritates me how they do this as well (refer to Fallout) i wish they go back to the old days and have weapons that never break or need cleaning/maintenance

If there's somekind of a video comlink between you and other people, you will undoubtedly see someone die while saying something to you. Kinda like "Don't trust *BOOM* AHHHHHH *ZZZZZZZZZZZZZZZZZZZZZZZZZZZ* Connection lost"

Minorities die first.
normal enemies take one shot and drop and then bosses take either copious amounts of lead into them or some special way to kill them... i dunno like shoot a piano thats over them.

Also as for http://project-apollo.net/text/rpg.html rule 185 is flawed seen a villain live happily ever after alot(well obviously not the main)

for RPGs:
The hero has a sword.
A bad guy that wants to destroy the world.
Your party of heroes is the ONLY party of heroes that can save the world.
No matter if you strip off all of your armour, you will always wear armour.
Magic.
You can create huge earthquakes and such, but you cannot open a small fence without a certain key. It's also impossible to just climb over it.
No matter how many times an enemy burns you with it's fire spell, your clothes and hair won't burn.

I could go on and on about this
