Definitely Handsome Jack from Borderlands 2. Some of the shit he pulled was worthy of certain assholes from Stephen King books, and that's saying quite a lot. Gearbox must've bred their writers to be gods because they crafted this character perfectly.

Wilson Fisk, the Kingpin, from Marvel Comics. He irritates the fuck out of me. First of all, he shouldn't really be around, the Marvel universe is crawling with superheroes who are able to eat his fat ass for breakfast, hell, he's even been dethroned by them on a few occasions. But he just keeps coming back, there's often been some vague justifications that he's a sort of balancing force on the criminal underworld, but that still doesn't make me like him. And the guy isn't even a fun supervillain or anything, he doesn't try to take over the world with death rays or anything, he's just a crimelord who's got his fingers in prostitution and drugs and is generally human scum. I dislike the idea that he's a necessary evil, and it all adds up to create a character who by all rights shouldn't be able to walk around the way he does, but is still around just because he's got plot armor.

For my it comes down to one thing: He's a Revolutionary, and like almost single one of them, he's a teenager at heart. Thinks he knows what's best and doesn't mind breaking the world without realizing what is being lost in the process.

I'm surprised this guy hasn't been mentioned yet:

This guy is an IMMENSE asshole. He tortures innocent people (women and kids included) in horrible ways in order to extract some sort of vitae from their blood, only to force them to consume an amnesia mixture so they forget the whole process, and then does the same thing again. Not to mention making his mentor a prisoner in his own body for hundreds of years, and convincing our fair protagonist Daniel to commit the same atrocities that he is, providing the false promises of safety from the horror that is chasing him, only to try and feed him to that same horror later.

You have no idea how much I enjoyed pushing over those pillars in that final chamber.

Cramming my way into the bandwagon and going with Joffrey. What a vile little shit sack.
Although I have an equal hatred for Cersei. Normally I love a powerful woman who takes what she wants, but Cersei is just evil. She is so relentlessly manipulative and just... every time I see her I burn with rage.
Kai Leng (ME3) was a terrible villain, and the only reason I hated him was because of his ridiculously stupid role and his fucking cutscene invincibility. Seriously, BioWare? The only good thing to come from that douchebag was his death. So satisfying.
More recently, the Governor in The Walking Dead. (From the television series; I haven't reached that point in the comics yet).
Seeing his initial interactions with Michonne was offsetting, but then watching him spiral into a depraved maniac was just... ruthless. When he finally got his hands on Maggie, that was it. I wanted to slit his throat myself. No one touches my baby girl!!

Not going to mimic everyone's favourite dwarf abuse target. I found I really despised Hoyt from Far Cry III, and I made that decision in a split second. Here's why - spoiler below.
During the final confrontation he killed Sam. I loved that guy. He was my bro. We shot up mercenaries together and didn't afraid of anything. Sam getting killed pissed me off... but that wasn't the real thing that tipped me from "I'm going to kill you because you're a dick" to "I am going to kill you because I hate you" it was near the end of the QTE dance.

Hoyt dropped his balls. He "taunted" me that I wasn't going to be able to save my friends. But it really was him begging for his life, being a coward, wishing that I - Jason Brody - would just up and leave. He thought I would just FORGET what he did. He thought I would run off to save my friends at the drop of a hat because he's scared. He imagined I would let him get away with this and just do it again to more people.

Vaas died better than you, Hoyt. Buck died better than you, Hoyt. Sam died better than you, Hoyt. You are the only one who is going to die in your own piss and shit and for that I hate you.

Just to give you an idea how much a monster Medusa is, she took her own son... daughte... child and turned it into a freak science experiment to bring about what amounts to the Anti-Christ. When Medusa first body was destroyed, she took over the body of a little girl, just so nobody would kill her again! Watch Soul Eater, and you will grow to dispise this son of a b-witch.

Oh god this. Hes a very convincing villain. And i LOATHE him.

When:

She jams that glass into his eye i roared "FUCK YES DIE YOU FUCKING BASTARD DIE DIE DIE" so loud people came over to see if i was ok. NOBODY hurts glen. GLEN IS A BOSS.

Kyoka Nanjou and Yakushiji Gaito from A Drug That Makes You Dream are my go-to characters for this. I would have found them to be utterly horrible people even without my involvement with Aeka. Kyoka generally used her as a way to keep her own position in her class' faux-hierarchy, a litmus test for seeing who else would fall in line, and a scapegoat to take out her own frustrations with her boyfriend, and her willingness to do so clearly showed her twistedness. Gaito was simply a bully and a violent thug and an implied rapist, and just the first glimpse of him and his sadistic, drawling voice provoked my fight-or-flight instinct.

Because I loved Aeka almost immediately (and only loved her more as the story progressed), though, I didn't just hate those two. They terrified me, with what they could and increasingly did do to my girlfriend and me. After a certain point during Aeka's route, I was starting each in-game day feeling honestly sick with fear. That fear, that loathing, and the throat-chewing rage those eventually combined into were part of what made the game so affecting.

...along with Aeka herself, of course, but that's more for a thread like "Characters you honestly fell in love with."

Yeah, I'll jump on the Joffrey bandwagon as well. His mother is also among my most hated villains.
Can't think of any others right now.

Also, someone mentioned Viserys Targaryen here, which I find strange, because I liked him.
Not as a person, but I would've wanted to see more of him.
