Everybody lived, on every play through. With an import from ME1 you don't have to do any planet scanning, and at that point only intentionally neglecting ship upgrades will get anyone killed.

I decided to keep the Collector's base as a "fuck you" to BioWare because I had gone through the game doing nothing but Paragon choices and I was sick of it. It was hardly "moral choice", it was just blindly picking the Paragon choice for "+4 Paragon", so I chose the thing that made sense to me which was to keep the Collector's base and see what we could learn from it.

But noooooooo, everyone at the ship was like "That wasn't a good thing to do! But you're our commander, so I trust you." Even the loyal Cerberus recruits (Jacob, Miranda, etc...) went "The Illusive Man can't be trusted," which I thought was supreme BS.

EDIT: Oh, and I only got "+7 Renegade" points for keeping the base. I really have to wonder how much of an impact it's going to have in Mass Effect 3.
I don't particularity mind any of the Illusive Man's methods, but I'm pro-alien, and I don't really care if 99% of human life is wiped out so long the majority of sentient life can be saved by the sacrifice. The Illusive Man directly states all he cares about is human interests. Given the option, I think he'd use the Collector base to conquer and subjugate all other alien races once the reapers are dealt with (if the reapers are dealt with). That being said, I did hand it over in two of my games, so we'll see.
Yeah, but if that ever happened I could just slap him a new one, though I doubt BioWare would implement that option. It was really annoying because I was trying to chose the options that I would do in that situation, but I figured out that there was absolutely no reason to be neutral as you don't get any benefits without being all bad/good. So, I just pressed the top choice every single time and it pissed me off. At the end, I said "fuck it" and chose to keep the base, and all of a sudden everyone became bipolar and went "I don't agree with what you did!" as if somehow my previous actions were null void.

So, yeah, screw "good and evil" gameplay aspects that try to push for "moral choices". I'm pretty sure those are two incompatible mechanics.

I don't understand why someone wouldn't upgrade their ship... making the wrong decisions during the Suicide Mission I can understand, but not upgrading your ship before you jump just makes no sense.

How the hell did you lose that much crew in the final mission. I only lost Jack on my first try and good riddance I say, she was a *****. The Normandy, Characters and most of the crew survived.

Zaeed wasn't loyal to me. So when I sent him as the first fire squad leader, he ended up getting Legion (the hacker) killed. I then used Thane as the bubble during the swarm area and as such Jacob was lost. Finally I lost Zaeed at the hold the line area due to him not being loyal. I'll admit that this wasn't my first time through and I was seeing what would happen if I choose Thane as the bubble (had Samara first time).

I lost Legion, Thane, Miranda and Zaeed on my first try.

2nd try, like most people, kept everyone alive.

All my crew were loyal except for Jack because of that assault on Miranda. I let Legion take the command of the second group with Tali and Grunt while I took Miranda and Thane. I switch Miranda for Samara and use Grunt and Garius for the buble section and again, I left Jack in charge while I took Thane and Miranda against the final boss. Final verdict, the only Character I couldn't stand was kill. The others are Safe and ready for Mass Effect 3.

My first two playthroughs, I lost nobody. My third, I didn't quite have enough paragon/renegade points to sort out the argument between Miranda and Jack, so I ended up losing Jack. That was fine with me though, because Jack has always irritated the hell out of me.

I've never lost a single person. I'll admit, that I used a guide to get past the suicide mission. Funny thing is, i'd have prolly gotten past it without losing someone anyways. I always get full ungrades and loyalitys... it's an ocd thing... I've have picked Garus to be my 2nd team leader, i always do, i'd use Tali in the vents, it just makes since, plus them two are my most tursted alies. Samara makes the most since for the biotic part, i'd have picked her, only iffy part of the whole mission would be who i'd send back with my live crew. If i'd have picked someone two powerful, i MIGHT have lost someone on the line... cause my end team would have been tali and garus. basically, if i'd have sent grunt or Zeehed back, i'd have prolly lost mordin... but if i sent nearly anyone else, they'd all live.

*shrug*

First playthrough nobody died.

Second playthrough I hadn't recruited Tali, Grunt or Legion so Thane died on the ship, Garrus was my tech specialist, he died, I sent the crew back to the ship alone, they didn't make it, Zaeed got carried away by the swarm and Jack died holding the line.
But the good thing is that I still have a Krogan in stasis on the Normandy, I'm gonna wait till Mass Effect 3 to open it, or maybe just until I get all the DLC

Made it through with no deaths (and no lost crew) without any walkthrough or outside help. I got screwed into being locked in the endgame for the first one with Vrimare and wasn't going to let that happen to me a second time, so I got all the companions and did all of their loyalty missions, fully upgraded the ship and made rational choices in the ending parts. Even then it felt tense, but pulling it felt so good.

I lost the Normandy's crew but everyone else survived the mission. While I can see the Illusive Man's point about the Collector base, I didn't trust him enough to leave it intact, so I'm probably not going to have Cerberus on my side when Mass Effect 3 comes around.

Same here, I was going to do her loyalty quest after Legions, but then my crew got abducted, so it was off to the Collector base.
I delayed my trip thru the omega 4 relay to do Legions quest, unfortunatly that meant most of my crew where turned into goo:/

Anyway... the only one that died for me was Jack - and that was on purpose (I.e. I made sure she didn't have loyalty so that she dies in the holdout when you fight the Human-reaper).

But seriously did anyone else find her as annoying as I did? From the get-go she had a stick up her arse and took it out on everyone else, she was ugly and barely wore anything (Blegh).

Glad I won't be seeing that ***** in ME3.
So, it's okay for you to hate on Jack to your hearts content, but anyone says anything negative about Tali and they earn an angry face?
Double standards sicken me.
Also for what it's worth, Jack is absolutley gorgeous.

OT: Anyway, I had quite a bit of time before I actually got to play Mass Effect 2, so I did quite well. I had obsessively studied and planned out the best possible solution and no one died.
In short, it worked perfectly and everyone lived. And I felt pretty awesome for it... although now that I've done it several times over I wonder if I haven't found a game breaker or if the Suicide mission is just that easy.

I actually really only wanted to save Kelly. She's the only female character that didn't irritate me. Well maybe Morinth..
Right now I'm trying to keep Thane, Garrus, Mordin, Morinth and Legion. The rest can go to reaper hell.
