In BioShock 2. I played for about 6 hours straight without saving. Got lots of stuff, did lots of things... and when I decided to save my progress, I accidentally selected Load instead of Save. I clicked the first save in the list and, suddenly, the game loaded to the exact same point in which I began six hours before.

It wasn't as bad as losing a 98% in Red Dead Redemption... but it really pissed me off.

Castlevaina SotN: couple 203% saves :/
XenoGears: like some 30 hours in
Parasite eve 1 and 2: end game and with some SWEET gear for PE1, just maxed out in PE2
Lunar 1 and 2: both end game saves, and with the best gear
Digimon World 3: maxed out the timer, and yes i played loved it, and was no where near done, LOTS of digivolution grinding (had Black WarGreymon WAY before i should have, it was awesome)
Mega Man 8, X4 , X5, X6, all again, end game
Soul Reaver: end game save
Gundam Battle Assault2: , 100% unlock file, not hard, but annoying to do over
Threads of Fate: both story's finished
Enihander: i'd say around 90% of the weapons unlocked, and all but one of the ships, pissed me off
Breath of Fire 3 and 4: all Genes and most the good gear in 3, end game in 4
Dark Stalkers 3: a custom Felicia, high level, and fully capable of slaughtering anything thrown at her
Legend of the Dragoon: end game, with max combo and Dragon Sprit levels
Ledgend of Mana: several repeat plays, high level
Omega Boost: clear file, on several difficulty settings

needless to say, i was PISSED, some games i haven't touched again since ( XenoGears and DMW3 mostly) due to the work LOST

If there's anything I got from this thread is that I should backup my Oblivion saves (though it might me a little late... I fear a mod for magicka might have effed up the game).

On topic, a GTA: San Andreas save, fairly complete, gone. Sadness.

In Radiata Stories, I made a human file and a non-human file. After my first five minutes playing non-humans, I saved over my human file and have not touched the game since.

Lost my game saves to Diablo after a computer crash. Accidently saved over Donkey Kong 64 losing about 70%. Lost Sphinx and the Cursed Mummy for Gamecube in an overwrite. Swapped out hard drives in my PS3 and lost about 50% progress on Stranglehold.

Dang, I've had a bad run.

My memory card with over 1600 hours played, all the trophies, characters and maps for Super Smash Brothers Melee got corrupted somehow. That was just play time logged in as me as it were, not the countless battles we didnt pick our named profiles on.

This was accumulated over about 2 or 3 years.

Still brings a tear to my eye.

So much work and time just....gone.

Back in the day of San Andreas, I'd completed about 75% of the game [with cheats].
The problem was, the game had a glitch that prevented you from completing the game if you input cheats more than 300 times.
Yeah, I was pretty damn peeved.
Still haven't completed that game.

When I was younger my brother dropped my PS1 memory card down the stairs and broke it. He thought it was just another little toy. I lost FF7, 8, 9, Tactics and Resident Evil 2 that day......

Mercenaries 2: World in Flames. I was at the last "boss" battle, just needed to blow up the bunker and kill Solano, but as fate would've it, my little cousin wanted to try out the game for himself, and the little twerp ended up saving OVER my file. It's been 4 months since, and he still isn't welcome at my place.
