The most recent one for me was in Shadow of the colossus. I was just riding around when I noticed a large pile of rocks in one area that hadn't been there before. Then I realized that it was the remains of a colossus I had killed earlier. That really got to me. I was thinking Wow, I really can't get away from what I've done here killing the 14th colossus was emotionally tough for me, too.

That was a good feeling. Also, some parts of s.links. Like when Rise broke down and cried in hers. I really cared about those characters.

Sadly, I've yet to do Rise's S.Link. I'm saving my second playthrough for a Let's play, so I only got the ones I did first run through.
But before you freak out about spoilers like you did with Dragon Age 2 and Merrill, it's cool bro. No worries.
Though near the end of Chie's one with the robbers, when she basically said to hit her or leave, that was impressive. And Dojima opening up about his wife, and eventually calling you family, that felt... well, it felt great.

Quite a few, but the most recent one would be towards the end of Twilight Princess when Ganondorf has by all appearances killed your companion Midna and crushes her helmet in his bare hands before charging you accompanied by five pale riders. If you liked Midna, this was an enraging moment. Too bad the fight is easy whether you're raging or calm.

When you find Dom's wife in GOW2, you think at least you find her in fair condition, that far from what happened.

It would have to be 95% of bioshock, and 100% of Fallout.

I played from 3 without having any knowledge of the backstory, and learnt it all. After playing 1 and especially 2 putting it into context, I had so many HOLY HOLY HOLY **** MOMENTS IT AS LIKE...

Mind, blown?

The bridge incident in SotC.
I was really shocked, I hardly realised until then just how much attached I got to Agro. Kinda amazing, considering I only viewed him as a mere tool in the beginning
And accidentally, though with an opposite effect, the bridge incident in Ico.
When Yorda managed to grip my hand to rescue me was probably the best moment I've ever had in a video game. Made me so warm inside. Very surprised, too, considering how until then you've been always the one rescuing her
And of course the endings of both games. The ICO games were pretty much the only ones which manages to make me cry.
