I don't consider 10/10 a perfect game, simply the highest honor that can be immediately given to a game. In this category I personally find:
Resident Evil 4
Bioshock
Portal
Painkiller
Prototype

Bussinrounds is right. A game can have a 10/10 even if it has flaws, as long as its other features make up for them.

No true 10/10 games, however the following would get extremely close to it in my mind:

Diablo 2
Resident Evil 4
Warcraft 3
Final Fantasy 6
Maniac Mansion: Day of the Tentacle

I can't find a single flaw in the original Metroid Prime. There are other games I would consider 10/10 in terms of overall experience or value, like Shadow of the Colossus or Civilzation IV, but there's always a couple things I thought could have been better or wish they did differently. But for Metroid Prime I can't think of a single thing.

I'm currently playing Zelda Wind Waker and I think it deserves a 10/10 so far so unless something makes me change my mind that, also Portal, I can't find any faults with, no it wasn't too short because it just didn't feel too short as opposed to say Beyond Good & Evil where I finished it and thought, "Damn that game was way too short"

I'd have to say BioShock and Mass Effect 2. They pulled me in like no other games. I've lost count of how many playthroughs I have in Mass Effect 2. I could probably play that game forever.

There are two games i can think of that fall close to the mark; Portal and Space Rangers 2
portal was just generally awesome, had no major problems

Space rangers managed to make a game that encompassed elements of turn based strategy, RTS, shooter, arcade games and even text adventures, without incurring major problems. The only problem i can think of is the robot AI in RTS battles can be thoroughly retarded. Generally awesome, fun game. Go play it

I totally agree with this guy, nice to see a fellow who also loves Civ 2, so many hours lost to that game...anyway, don't think a 'perfect' game exists, games are somthing different to everyone, my favorite game is Morrowind, but i wouldn't say it was 10/10...

Super Mario Galaxy 2. I have not a single complaint to rally against it. Sequel that's a remake? So what, it's a sequel that used the ideas of the first better than the original.

My personal 10/10 game is Mass Effect 2. Story, controls, graphics, gameplay... all top-notch in my opinion.

Can't think of another game I'd give a 10 to...

Obviously the question is not what game deserves a 10/10, its what are YOUR 10/10 games, your all time favourite, the ones you will never forget, and never get bored of, and probably play it through once or more, at least every year.

Mines are:

MegaMan 2(NES)
MediEvil (PS1)

I can't think of a game that doesn't have some flaws, or at least some things that could have been done better. So I guess 9.9999 is the best I could ever rate a game if I were to be honest with myself.

And the only recent game I can think of that would get such a high rating would probably be Red Dead Redemption. When looking back on it, just about every element was hitting its mark in my eyes, but when I look back on it, something so basic (that I just adapted to over time) like movement off-horse could have been done much better. I know Yahtzee hit on that, and it really is my biggest issue with the game at times. Seems like pressure sensitive analog sticks are meant for resolving issues with movement speed.

One of the reasons why I liked Undead Nightmare so much, where it was nearly impossible to walk (which was a good choice)
