Horrible combat system, and a storyline to boot. I was young, foolish, and looking for a decent RPG.. GIVE ME BACK MY DAMN MONEY!

But to answer your question: I believe a bad game is made when the plot sucks, the voice acting is terrible, and the characters are uninteresting. Also, anything with a movie liscense usually sucks johnson.
EDIT: Except for Butcher Bay, which will always rule. Always..

Yeah, basically this. It's not rocket science. I think the cause of it all too often is developers focusing on graphics first, especially nowadays.

Most of the things that make a bad game are actually obvious within a few minutes of playing it, which sometimes leads me to wonder is anyone play tests some of them.

Fetch quests, so-called 'moral choices' that are as black and white as a zebra and it seems that most FPS's have magical regenerating health as long as you hide somewhere for a minute, destroying any challenge the game had.

If the game makes me feel like I'm doing a chore more than having fun, then that's a bad sign.
If I have to keep looking at the manual to check the controls, that's also a bad sign.
The story could be campy as hell, the voice acting could suck, the music could be ear-jarring, the graphics could be old-school, and I would still play as long the game was fun and had intuitive controls...just with the sound off (and my own choice of music on) and not paying attention to/taking seriously the story.

Horrid controls and unresponisve camera
Sucktastic story, dialogue, voicing
Sounds that make you wish you were deaf
Easy exploits in the game
Glitches that prevent going further into the game
One where you need a guide, not the 4 page instruction booklet which is a pet peeve of mine, just to understand the basics.

poor story, bad graphics, terrible voices, short gameplay, bad physics, unrealistic difficulty, no human genicide and FUCKING QUICK TIME EVENTS!!!!!!!!!!!!

Bullet lag (rainbow six)
Teamkilling (every FPS shooter)
Horrible hit-detection (EA sports games, Battlefield Bad Company)
Laggy servers (Call of Duty 4)
Story mode is the exact same as multiplayer (Left 4 Dead)

Now, I love Rainbow Six, Battlefield BC, most EA sports games, and Left 4 Dead is not a bad game. Those things listed above are just things I think are what makes games bad.

im not going to rehash what people before me have said, but no replayability definitely adds tremendously to my dislike of a game. For example. God of war. Great game. you get to the end and can't do ANYTHING. only way to play again is to start a new save file, you lose all your upgrades and shit. that 4-5 hours of awesomeness does not compensate for 50 dollars you pricks.
