Fake difficulty (e.g. AI getting ridiculous buffs and cheats but not getting any smarter or more skilled), game breaking bugs in favour of the AI characters
As I just told someone else, halo 4 was really bad about this too. The higher the difficulty, the less effective your weapons are. I actually shot an Elite in e head with the Binary Rifle and it just removed his shield but in return getting hit anywhere by one is an instant one hit kill. I think things like that are my biggest pet peeves of all.

Games that are nothing but endless set-pieces and cinimatics with only monotonous shooting in between. I don't even know how these things can be classified as games any more, since there is no 'game' to be had anywhere here.
While I agree with the point being made about the problem games have, your definition of "game" is a little faulty considering the first computer games were things like Space Invaders, Pong and PacMan. Games that had an analogue stick and one or two buttons and were literally a case of "Once you have played this game for 5 minutes you have seen all it has to offer."

Mostly players who are not new to the game breaking the unspoken rules of the game. though there are a lot the most glaring rages come from LOL and the dicks the make up such a large part of the community. Of course trolls suck, but it's usually clear what they are doing in the team pick and usually there just playing to have fun. What gets me are the people who are just playing for themselves, ones that don't communicate with there teams in character pick and just do whatever they want. And even that's ok somewhat as long as they call or pick a character first, but if not it makes me mad.

Sports games that just "decide" when you're going to lose or not.

When you bomb that pass into a receiver and it bounces off his hands, into the air and into the opponents hands and THEN that guy breaks 4 tackles to return the ball for a touchdown with 1 minute left in the game....

Shit like that...the game just goes "Nope, I win, I'm the computer, I can do whatever I want", that induces rage.

Modern video games as a whole infuriate me, and the communities behind them are even worse. The fact that "gamer entitlement" is something someone can say without being ridiculed by hundreds of people is nothing short of fucking disgusting. Standards have fallen as well. A lame, lazy, cashed-in joke of a game like Borderlands shouldn't be winning any awards, and the fact that so few people recognize Heavy Rain as one of the worst games of all time(which it is) makes me physically sick to my stomach.

When I don't know why I lost. Sure losing a lot by itself can be frustrating but its even more so when I have no idea what I'm doing wrong.

Ladders.

I have never, EVER played a game where ladders have been handled well. I look up and push down and I climb down, then if I look down to try to see what I'm doing, I CLIMB BACK UP. And I can never get down a ladder, I walk over towards it then end up falling down it to my death.

Also full-game crashes. Listen, I can understand that games are hard as shit to make, BUT I DON'T CARE. There's no reason that your game should require a full system restart because it fucked up. EVER. And it only seems to happen with games nowadays. I could sit down and play Roller Coaster Tycoon for the next 4 years and I know it would never fuck up.

Modern shooters. Particularly ones that try to be COD, or appeal to COD's market. I know you want to make money, devs, but if I want to go play something like Call of Duty, I'M GOING TO GO PLAY CALL OF FUCKING DUTY. Everybody else, the casual market included, isn't going to buy your game because it's so much like COD. If you make your game Call of Duty, I'm not interested, because I've already played Call of Duty and by extension will gain nothing by playing your game. I purchase games because I'm looking for something new and different, not a slight rehash of what I've already played.

Honestly, it's not so much games these days as it is gamers.

I fucking hate the way a lot of gamers act these days.
I honestly think a lot of it is people have always been that way but everything is so connective we actually notice now.

I dislike games with repetitive and graphically uninteresting areas. It just gets really boring for me after a while.

Well i have a small list.

GFWL that annoys me to no end when added as you end up with so many dam errors half the time you can not play it.
PB you have to be a hacker to get the dam thing to work most of the time.
Lag the most annoying thing ever go in for the kill shot or about to mirco to win a RTS lag = dead.
Lack of balance be it units in RTS or say magic skills or weapons it just becomes annoying.
Random Crits in FPS style games ruins the balance.

I can deal with the rest of the stuff they put in that other people find annoying but there the big one's that annoy me.

Gamers who don't understand core concepts about the fighting games they play. IE throws aren't cheap, zoning isn't cheap, that combo isn't an infinite, it's just optimized as shit and means defense is an extremely important part of the metagame, the fact that defense means more than just holding back (being prepared for tick grabs, 50/50s, ambiguous set-ups, option selects, etc). The notion that I suck at the game (after I beat them, no less) because I didn't play in a way they liked (as if I'm playing the fighting game for this person to have a blast). I just miss playing in arcades, where getting bodied meant you wasted 50 cents, a couple of minutes, and salty losers most likely weren't brave enough to talk shit to your face.

I think the thing that makes me most angry about any game is when it's boring. Boring like I expect the tropes, or the combat is just hella-uninteresting, or the characters are unlikable. I'm more likely to enjoy the hell out of games that I don't think are all that good (Resonance of Fate, Recceattear, Thomas Was Alone) than I am likely to enjoy games that ought to be good but are boring as all hell (Diablo 3, Ni No Kuni, Dragon Age: Origins (although to be fair, I really like the characters in DA:O, I just loathe everything else)). Being boring is the cardinal sin for me. Be stupid and buggy and weird all you want, but if you make me yawn, then I'm probably gonna hate you.

I hate it when I play games that let me make custom characters, but for some reason do not allow me to make an brown skinned person. Dragon Age and Kingdoms of Amalur: Reckoning( the demo at least, never got the game) are shit about this. Kinda bugs me in the Fable series, but to a lesser extent.

Lag. Anyone who plays online games should have this for their top answer.

"Artificial" Difficulty - I'm not talking Dark Souls. I thought that and Demon's Souls was punishing but fair. I'm talking something along the lines of enemy AI being damned near psychic. Perfect Sims and Dark Sims on Perfect Dark, Veteran Difficulty on CoD are some examples.

Infinite combos
Infinite combos should not exist. I was playing Anarchy Reigns online, and mixed with the lag I had, I could never break a combo, and consistently getting knocked into a long combo by someone who on my side was clear across the map. Lag and timing issues made combo-breaking impossible, if it was in the first place.

Open PvP in MMOs
Let me level in piece you bastards. I played WoW for about a week on a private server that had open PvP. It was even modded so that only Shattrath(sp?) City was safe. Needless to say there were "Donors" who had 3000 or so of EVERY STAT (Donating gave you free T6 gear, and special gear that gave 200 to every stat for EVERY SLOT, except trinkets) And of course people camped the starting areas. Freaking lvl 70s.

I played longer on the real servers and played the last expansion (Cata, not Panda-land) on a PvP server. I ended up getting camped an entire guild when I was level 50 or so. Most of them were level 85.

UNSKIPPABLE CUTSCENES: I can see forcing the player to watch it once, or if it's masking a loading screen (Show me a progress bar if I try to skip one of these, please)

Verbose Tutorials: I've been a gamer for over 20 years, I know some people are starting their first game while this is just another in my collection, but you don't need to treat me and every other gamer like they are pants-on-head retarded. I don't need a dialog box telling me how to move or "Press X to when the "Press X to use" icon shows up, which is ALWAYS shown as, who'da thunk it, an X, and usually a caricature of the PC doing an action, or even words.

Other (Online) Gamers: I like multiplayer games with friends. WITH FRIENDS. I abhor the average online gamer though. whether it's the elitist "pro gamer" who apparently sleeps with everyone's mother, the trolls, or the people who want to get good at a game without any effort (I saw this a LOT in WoW. The game is not hard to be decent at. I figured it out and was usually the top DPS in any run I went on. I had one person who CONSTANTLY bugged me about how to lay because we played the same class. I linked him to a site I used, and he complained that he didn't want to read anything, he wanted me to teach him... this was through the in game chat too.

-Generally, games that tout that the game is super immersive and engaging, then a big, giant QTE button appears in the middle of the screen. For me, nothing breaks immersion faster than that.

-Games that require 5% skill and 95% luck.
Every Mario Party game ever made has been made like this.
It pisses me off to no end when an opponent beats me by 1 star because he rolled a six-sided die better than me.
And the games just got worse as they went on, in one of the newer ones there is a mini-games where you have to pick the tallest tree, but no one knows which tree is tallest until everyone picks one (yes, they literally made a mini-game where everyone draws straws.)

I feel like I could just mash the A-button without looking at the screen for the entire game and I would have just a good a chance of winning as everyone else.

Idiot AI partners, especially ones that get in the way. Colonial Marines has this one in spades. It feels like everytime I'm lining up a shot one of those morons steps right in front of me.
This. On my sneak archer in Skyrim I kept accidentally killing every single follower I got because they though stepping in front of my best enchanted bow with the best enchanted arrows while I'm sneaking just as I release the arrow is a great idea.

Also the whole DRM thing. If I buy a game in Steam, that should be enough, but no, I then have to get U-Play, or Games for Windows - LIVE or some such dumb crap. And now I can no longer get new EA games on PC, because that means Origin. Too bad, I do want to play ME3, but I can't be arsed to get it on my 360 and lose what I did in ME1 and 2, and I don't like Origin. So yeah, that.

Captcha: Finagle's law. Even captcha agrees that the timing of EA insisting on Origin is bad.

I have grown to despise the majority of online gamers I come across. Namely when they gloat about how well they've done or are doing despite using something like the the easiest fucking weapon in the game or the most pathetic method. The amount of times I'll be playing with my friends and they'll mention how much better they're doing, only to turn out that they're camping with a shotgun or something.

For instance, the amount of times my friends will go on Conquest on Metro in Battlefield solely to use the Striker or USAS and then gloat about the amount of kills they get. WHAT YOU'RE DOING INS'T SKILLFUL IN THE SLIGHTEST!!

...otherwise, when bosses regenerate large amounts of health moments before their death. Or after defeat, show up later in the game, EVEN STRONGER!

I notice that I get very quickly annoyed by poorly optimised gameplay (great example: the spawnsystem in CoD), and something not neccessarily related to gaming, my relatively slow connection. I hate it when someone kills me who is not exactly better, but just has a big advantage because he has a better connection.

This goes doubly for anyone who plays world of tanks. COD players will just mock you or are chavs who have picked up a controller. World Of Tanks players will spend the entirety of a match shouting at their team for not helping them (these are 15 minute matches) after suiciding.

Anyone who can spend 15 minutes frothing at the mouth isn't right in the head.

Earlier I mentioned online-only multiplayer being used in games that talk up their multiplayer to no end. I also griped about bot matches not being in anything anymore. Here's another thing that's going to really piss off achievement hunters in the not too distant future: Games like Halo 3, Mass Effect, etc with achievements based around DLC or online play will be impossible to complete in terms of achievements. I kind of gave up on achievement hunting long ago but the fact that I'll never earn many achievements because servers will be taken down and DLC will be made unavailable outside of the possibility of a complete edition of games with said DLC. Not so much of something that enrages me as it is a problem I have with achievement hunting and game completion...I was a lot angrier about this back when Halo 3 launched and I realized there were a shitload of achievements exclusive to online multiplayer.

It's been said but I'll reiterate it, artificially raising the difficulty by making your character weaker and your enemy stronger with better armour and weapons. GTA had it right by making the game progressively more difficult by throwing more enemies at you and generally putting you in more difficult situations. That's how it should be, or if you do want to raise the difficulty make the AI smarter by giving it more paths, options and algorithms to work from.
