What games have you beat on the hardest difficulty?

Very few, and it's been a while. I really have to love a game to play through it on the hardest difficulty. So...

Doom/Doom II - Ultraviolence (not Nightmare)
Fear Effect 1 and 2
Red Alert
Civilisation II - conquered the world on Deity.
Disruptor

Of those I'd say Civ II was the hardest, due to the AI being insanely agressive and blatantly cheating.

Funny thing is, once I've beaten a game on it's hardest settings I never go back to the easier/normal settings because it becomes too easy.

Silent Hill 2 - Hardest puzzle difficulty, not combat
Bulletstorm - This was relatively easy though
Deadly Premonition - Laughably easy with the secret weapons

I also did Fallout New Vegas on hardcore, but left the difficulty at normal so that doesn't really count.

I prefer to play games for their narrative as opposed to the challenge, so I generally play on normal difficulty.

well I beat the subspace emissary on intense. but I don't know if you would consider that too difficult.

wait does the very last level in super mario galaxy 2 give me any points? cuz that was pretty damn hard.

heh sorry I'm not JUST a Nintendo gamer and I KNOW there's something else but i'm having a major brain fart right now.

I only wanted to see what hard time attack had in terms of items.

If the game is survival horror or hellbent on being realistic [or as realistic as the fictional universe will allow] then I tend to play on the hardest difficulty. SH: Homecoming was still incredibly easy, though. Pisses me off.

only counting 360 games here... i wouldnt be able to remember the hardest difficulties ive beaten on previous consoles, other than Halo 1.

Call of Duty 2, 4, 6. Halo 3, ODST, Reach. Splinter Cell: Conviction, and HAWX. i may be missing a couple, but i dont play on the hardest difficulty very often. i usually just play either easy or normal, just so i can experience the story of a game.

Due to Trophies, I usually beat games on the hardest difficulty. I dont do trophies so much these days, but I still play hard most of the time.

the last level of CoD 4 on Veteran. is. the. Biggest. joke.
Im serious, it's ridiculous! i think it's called the Mile High Club, and it's ridiculous.

i mean the halo 3 vidmaster achievements are hard but whats even harder is to get a bunch of mates to accomplish it with.

Halo Reach - The beginning bit where you fight elites for the first time. It took me and my friend like an hour. It's weird because I beat it easily when I played on single player.
MW2 - The top of the oil rig. Beat it on PC but could never beat it on Xbox.

Other than tat I can't remember what games I played the hardest difficulty on.

TIE Fighter, X-Wing vs TIE Fighter, Wing Commander 3 (the first two WC games didn't have a diffculty setting, I believe) and Mechwarrior 2, 3 and 4. Oh, also Crysis and Crysis Warhead.
They're the only ones I can definitely recall beating on their highest difficulty. Probably some others, but I don't remember. I like piloting stuff in games and I tend to be good at it. That's probably why I miss the space sim genre so much. Most non-sim games I tend to play at medium difficulty.

Shadow of Chernobyl comes to mind, as does Doom and Marathon 2, but beyond that, unless difficulty alters the gameplay mechanics in some way I usually don't bother. I've beaten New Vegas on Hardcore, but not on Hard, for instance because while Hardcore boosts the actual difficulty and adds in a bunch of new systems, Hard just turns everything into bullet sponges.

Only counting current gen games on the 360 I've completed on hardest difficulty...

Call of Duty: Modern Warfare - Mile High club was HORRIBLE.
Modern Warfare 2 - I can only remember the Veteran Breach and Clear mission on the oil rig being a dissaster in special ops mode. :(

Halo 3 - Completed on solo legendary.
Halo Reach - Completed on solo legendary too, hardest level was likely The Pillar of Autumn trying to fight my way to the MAC Gun.
Civilization Revolution - (Okay, well... Completed in the sense I have 1000/1000 including the achievements for victories against DEITY level computers. Quite prowd of myself for this one really.) Hardest level was likely getting a Cultural victory considering all enemy wonders construct at half the cost and there were no more great people to get at one point. Had to send my spies on massive kidnapping missions to aquire Einstein and Aristotle.
Command and Conquer: Red Alert 3
and

Dragonball Z: Burst Limit - (SO MUCH RAGE was involved in completing certain levels.) Specifically fighting Freiza with Goku when Freiza gets all the good drama pieces and starts on true form while Goku isn't even in Kaioken.

Super Meat Boy - The Dr Fetus boss level and afterboss level were painful... Just no game. Just... Stop. Couldn't bring myself to try and complete other levels afterwards.
