Bioshock (I think the PS3 got an extra difficulty level + no vita, also i swear the difficulty curve is backwards)
Infamous 2 (not that hard when evil)
MW2 (way too easy)
ME (Hard, but only if you play stupidly, for the most part)
CoD5 (heat seaking grenades, FUUUUUU)
Gears (The final boss was bearly bigger than normal yet took 100's of bullets and explosions)

Can't say I've beaten the whole game but I got through most of Hitman Bloodmoney on Professional difficulty - however, the easiest level will always be the casino level - takes six minutes no matter what difficulty it's on

Last week I beat Diablo 2 on Hell difficulty for the first time. I'm not much of a B.Neter, so I usually quit after Nightmares

Civilization Revolution - the hardest to beat because that game has no shame in cheating to win.

There are more for PS2 but those are all the prominent ones on the 360/ps3 I can remember.
Favorite one on PS2 was The Suffering's "impossible" difficulty. Enemies had no impact shock from being shot so I went to town with the shiv and fire-axe the entire game, hell of a lot of fun.

inFamous, relatively easy

Modern Warfare 2 (The only level I had trouble with was Loose Ends. I got a horrible autosave point where I would almost die immediately while trying to escape the house. It took me HOURS.)

Mass Effect 2 (I was an engineer on my 1st insanity run. I had no trouble throughout the whole game that playthrough, but for some reason I had more trouble during the Horizon,
Collector Ship, and Reaper IFF missions
as a sentinel.

Call of Duty 4 (I can hardly remember any levels from that game, but I remember the first Paul Jackson mission gave me some trouble)

I beat New Vegas on Hardcore on my 1st playthrough, although that doesn't count because I still had the difficulty on normal.

I tend to do the 2nd hardest difficulty on the first playthrough, then the hardest on my next. Because too often the hardest difficulty is just a grind that would turn me off of the game.

The hardest GAME I have ever actually finished is Metro 2033...

Playing that on the hardest setting is something akin to the 1992 Mario games...
Where you spend 30 attempts at one area just to FINALLY get past it and hit another area with the exact same problem... Albeit no quicksave does make it quite annoying between checkpoints, but, I found it the hardest MODERN game I have ever played....

As for OLD games, the aforementioned Mario comes to mind, but actually, an old DOS game takes the cake...

Abuse from 1997 (which is now free-ware, so, look it up if you want a challenge) is one of the very few games to have defeated me.....

I wasn't even able to pass 50% of the game on "Extreme" difficulty (hardest) so, if you're looking for a challenge, look it up......

I've completed like 2 games on Hard or Hardest difficulties, I usually play games for the narrative though, not to get constantly frustrated.

KillZone 2 - That last part just before you kill Scolar Visari's General took me days to beat
-COD MW2 - Cant remember the hardest level, they were all about the same.

I got pretty far on Uncharted on Hard. Then I got to the church level near the end and thought "Fuck that shit." For most shooters I usually don't play on hard because I just want a brain-less manshoot.

Notable games I've finished on the hardest difficulty were Tony Hawk's underground (the only I've played that had difficultly levels, still fond it a doddle.), and F-zero Gx, every cup on master, every story mission on Very hard.

Hardest part of that one was definitely the story mission where you had to do a long flaming tunnel full of obstacles in 50 seconds. I finished it in 49.75 or some such, there was literally no margin for error, I had full speed the entire time. It took hours...

Anyone who says CoD4 or MW2 is hard clearly hasn't played CoD2, that game is ridiculously difficult at times.

Most difficult game i've completed had to be Unreal Tournament 98 on the second hardest difficulty (couldn't do the first), Xan is a mo fo when he wants to be and so are my team when they decide to do nothing in the assault game type missions

Seriously? I find Prototype frustrating as all hell on the medium difficulty setting. Props to you, you are a better gamer than I.

Anyway, the only game I've ever beaten on a really high difficulty setting (Where it actually felt difficult) was Uncharted 2, on crushing. I've beaten Saints Row 2 on Hardcore difficulty, but that's only slightly more difficult than the default difficulty in other games.

I beat the SNES Sparkster on very hard mode (although it open up to crazy hard mode which was just impossible).
I also beat Timesplitter 2 and 3 on hard mode as well (if I remember correctly).
I can't remember I think I got on the last level (Ruin) on Phantsy Star Online on Ulitmate mode but I don't think I defeated the boss on that mode (it was way too tough).

The hardest mission, even on easy, is either 'Falling for Kafka' or this one bit, i don't remember what it's called, but it's in a giant walk in freezer.

Not counting straight up fighters or arcade ports with infinite lives, these are the one's I remember.

Duck Tales (GB & NES) (Hardly the hardest game in existence)
Flashback (SNES & PC)
Mass Effect 1-2 (Pinnacle Station + Hunt mission + Insanity = PAIN)
Mega Man 2 (I still can't beat that one boss without dying and returning with more ammo)
Metal Gear Solid 1-2
Rescue: The Embassy Mission
Resident Evil 4 (PS2 & Wii) (Unlocking the Handcannon was easily the hardest part)
Teenage Mutant Ninja Turtles IV: Turtles in Time

Both Bioshock 1 + 2 with no respawning. Not actually as difficult as it sounds.

Gears of War- the hardest part might just be the scene where you have to alternate between driving and killing krill on the old highway.

Gears of War 2- there was little hard about it.

Civilization Revolution- Match ups on deity difficulty, with Cultural being rather tough. Such a fun game though.

Borderlands- again, no real difficult sections.

Fable 2- great game, no that difficult outside of collecting all the dolls. I ended up heping people get some of the harder achievements for a laugh, eventually I helped over 100 people.

Call of Duty: Modern Warfare I & II: first one was actually rather tough, 'one shot, one kill', 'mile high club' and the SA Smission going down the hill are some of my most frustrating yet worthwhile achievements. MW2 was much easier.

Bioshock- early on is the most difficult part.

Some others games I can't thinnk of now.
