The Shalebridge Cradle level on Thief 3:Deadly Shadows.

That was the creepiest gaming level in history surely.

The original Silent Hill still scares the pants off of me. F.E.A.R. too - that game will NEVER get old, and I cannot wait for the proper sequel. Dead Space has kept me jumping and on my toes so far - it's doing its' job

Ocarina of Time. Say what you want, that game is scary.

Oh and the obvious Silent Hill. I bought it ages ago and I still can't brave the school. (I will complete it one of these days by the way...). I ended up watching cloud8745's walkthroughs of all the SH games on Youtube and I love the series, too bad I'm too scared to play any of them...

The Oddworld series can be pretty creepy as well.

Never really been on many 'scary' games, but the old House of the Dead series used to freak me out when I was young. Also, the zombie levels on Timesplitters 3 were quite atmospheric at two in the morning in the pitch darkness.
Others that scarred me as a child were the original Resident Evil and the Alien Trilogy...

To this day, the green things in Duke Nukem 3D that crawl on the ground and ceilings and latch on to your face scare me.

The end of Doom freaked me out sufficiently when I saw it for the first time. Man, I still can't listen to that song without getting unreasonably freaked out.

Fallout 3 manages to make me jump when I'm in the middle of a firefight with some raiders, and a Yao Guai or hell, even a Brahmin that I accidentally shot earlier jumps out at me from behind.

The Regenerators were the only part of RE4 that scared me. Oh my God, who can forget the first time it hopped off the ground and buried its fangs in your neck in less than a second?

This is a little more obscure, but does anyone remember The 7th Guest on the PC? I think it was the first game that really scared the pants off me. The painting at the top of the stairs was what really did it for me. It looks silly by today's standards, but the entire mansion was nightmare fuel for my impressionable 3-year old mind.

Anything where I can die. I'm a total wimp, the original Prince of Persia gave me nightmares, the creepy skeletons that wouldn't die...

The Suffering (specifically the second one, "Ties that bind")
The game has things that flash on your screen, messed up creatures that pop out of no where, and blah.

I did get used to it after a while but that is because for some reason I kept playing the first level a few times. (I can't quite remember why anymore)

Not many really. I was freaked when the dude gets disemboweled(sorry about spelling) in Bioshock was scary........................................................... oh and...

Surprisingly...Condemned never truly scared me. I was kind of saddened by that. Same goes for Dead Space. I used to be terrified of the Resident Evil games and Silent Hill games when I was younger, but it seems I've grown out of it. As of now, nothing really gets under my skin, but when I was younger, Resident Evil 2 did the trick for me =X
Condemned didn't ever really manage to "scare" me - though it did startle me from time to time. Dead Space rarely even startles me, probably thanks to the combination of being incredibly resiliant to damage and being armed to the teeth (a note to game designers trying to scare me: don't arm the player well if you want them to be scared).

Doom 3 managed to be scary at moments, especially if you were low on health and out of chaingun/plasma rifle rounds. Silent Hill managed to build a wonderful sense of dread as I played. Clive Barker's: Undying had the scariest first level I've ever played in video games.

Firefox says spelling's all good!

Bioshock was creepy more than anything, especially when you first get the revolver. You know what I'm talking about.

When I was a kid "trespasser" always freaked me out. With all those dinosaurs that would charge out of you at no where. I haven't really played a game just like it since. I guess they decided it was a little to fun. It probably didn't help that I had a friend who was skitz beside me constantly freakin out telling me just to hide up on the monorail and await hunger and starvation take its natural course on me up there.
Years later Doom3 would scare me. And a while after that the movie would disappoint me.
