Toxic Knight: Your days of rolling around in toxic, radioactive goo has finally paid off. You've become more resistant to radiation, and have learned how to harness it as a weapon by slathering your sword with the goo. You gain a 25% boost to radiation resistance and as long as you're irradiated, your melee weapon gains a damage over time effect. The more irradiated you are, the more potent the effect. Requirements: Level 20, 80 melee, 8 luck.

Ghoul Crazy: You've always preferred the company of Ghouls over the company of Humans, now Ghouls prefer you as well. Get extra conversation options with all Ghoul NPC's, feral Ghouls are less likely to attack, Ghoul companions get extra damage, and female Ghouls want your bod. Lvl 18 CHR 8

[EDIT] Also, the picture for this would be the Vault-Tec Boy giving the thumbs up, only a ghoul version of him.

Knife to a gun fight: When using a melee weapon against a ranged opponent, you have a chance to deflect the bullet. Melee weapons:60 Luck:8

Just like Indie: When facing a melee enemy with a ranged weapon, you have a chance to score either a crippling torso wound or kill shot. Guns: 60 Luck:8

Seat of your pants: Sometimes things just don't go your way, whether it be poor planning or just bad luck, but somehow things tend to work out alright. Enemies have a chance to jam their weapons, they can unjam them, but it takes some time. Luck:7

I do say that that would make the game more fun. The Pitt addon has a bullet factory in it that serves the same purpose.
Edit: I do say that a lot of these have both good names and purposes. Someone should put this on the Nexus and see if anyone is capable of doing this.

Medic: Stimpaks do more healing to you and any followers.
Short Fuse: Grenades may explode in an enemy's hand mid-throw.
Wasteland Knight: Wearing armor of a certain faction will get you more acquainted with them (i.e. more supplies, better weapons)

Perks

A Real Soldier: You're loyalty to the NCR is unmatched. Any NCR troopers near you gain combat bonuses.
Acquired by: Gaining enough NCR loyalty

The Final Frontier: You've explored the Mojave an seen some of the most ugly mother f***ers out there. The best way to kill them: with the biggest bug zapper you can find. You gain a 75% damage increase against mutated creatures with energy weapons.
Acquired by: Level 20, Science and Energy Weapons at 75

Ace of Spades: You hold all the cards. In addition to an 8% critical hit chance you're enemies suffer various bouts of bad luck (guns jamming, dud grenades, etc...).
Acquired by: Level 25, Luck 8


This is just an idea based off FO:Tactics. At some point in the game (or multiple points, either or) you can change your race based off previously used ones: ghouls, talking deathclaw (yes, some talked, look it up), robots, and super mutants. Since most humans are bigots in FO (says a lot), you lose 1 charisma for a race change.

Here're my ideas:

-Open Season: Small Guns skill boosted by 20, Small Guns /now do increased damage against animals. Lvl. 14 and Agility of 7 required.

-Sunburst: Energy Weapon Ammo slowly regenerates when standing in sunlight. Lvl. 21, Intelligence of 7, and a Science Skill of 60 is required.

-Jedi Mind Trick: New conversation options appear in dialogue, all speech checks now much easier to pass. Charisma of 8 Required.

-Dead-Eye: All Pistol-type weapons now have 50% reduced AP costs in V.A.T.S. Perception of 7 Required.

-A Monument to All Your Sins: Players can frighten enemies. Most enemies now have a 1 in 7 chance of either running away from battle with the player or switching sides to aid the player in combat. Lvl. 25 and a Karma level of -700 required.

-Clayface: Players can now disguise themselves as an enemy, provided that they have the correct armor.

-Kryptonian DNA: Strength, Agility, and Perception increased to Maximum. The effects of Radiation Poisoning are now 3 times as severe and also cause the player damage when they come in contact with radiation.
