what is the best/worst gaming peripheral?

Best - The Guitar Hero Controller, sucha brilliant design. Worked so perfectly.

Worst - The Wii Fit Board or the Eyetoy...both had little use and barely worked.

WOrst peripheral I ever came across was the "thrustmaster Fragmaster" Joystick:

Yeah seriously. A two handed two axis joystick, for playing Quake in 2000.
It was awful. I didn't buy one, we stocked them at the shop I worked at. As far as we could work out, it's only use was to handycap a really good player so everyone else could kick their arse.

Anything from sony, I've yet to see a properly working sony product (except for 1 single PSP and it got broken by some jackass who was screwing with my stuff). Then again I have bad luck with that kind of thing.
And no I've never tried a PS3 because of this.

AC6 Flight stick for the Xbox360 Now that was way better than the real controller for what it
was meant to be used for.

The eyetoy was pretty bad, but it tried its best.
And the best one would obviously be most of the GH/Rockband Gear.
