Tried to get all of the hidden challenges in Red Faction Guerrilla online, but I stopped because there was no way I could get the one for playing a match on all of the maps in that game.

Did the obligatory 'hardest mode' run on games like InFamous, The Force Unleashed 1(uh) and 2(damn short).

Beat Crash Bandicoot 2 in one day, beat Crash Bandicoot Warped in one day, then got 104%, then did it dying only 4 times (boss fights didn't count( yet)), and beat Crash Bandicoot in one day.

In Unreal tournament 3 I played enough matches using only one weapon to get all of those trophies.

Currently trying to get the trophy for reaching level 60 in MAG playing as Valor, and I will do so even if I loose my voice yelling at my teammates who I swear are operated by early PS2 era A.I.

Impending: One that I'm going to do will be doing an entirely non-lethal run in Dishonored.

Mass Effect 2: Tali's Mission on Hardcore with Jack and Jacob. I still have nightmares...

Thankfully, on my Insanity Run, I had some help- the DLC weapons onmy teammates, which are just as effective on your mates as they are for you.

No, seriously- get Zaeed and Garrus, give them Incisors/Mattocks and you can blast through the game until Horizon on Insanity.

I would have to say mine was finishing "Demon's Souls" playtime in total was about 130 hours but I am pleased to say that I managed it

Enslave every single non-essential person in the Wasteland in Fallout 3. I've cleared Megaton and Paradise Falls itself :U

I gave myself a year to 100% Super Meat Boy. I got maybe 50% . At least I beat the light world.
And I got all orbs in Crackdown 2 except the online ones without a guide
EDIT: beat all "run through" achievements in Mirrors Edge ( no guns , Hard ) I also tried to do them at the same time afterwards

Interesting, tell me more..
Here's a few examples I can think of at the moment:

It is extremely important that you let all the animations go all the way through whenever possible. That is to say for example don't cancel the land animation with movement. You could use your dive attack shockwave to instantly destroy a group of guys or you can casually land in the middle of them all and watch them freak out cuz hey it's Batman!

It is also important that whenever there is one guy left you turn your back to him and walk (not run) away as if you didn't notice him. You then do a backwards counter for maximum coolness.

As a rule walk > run whenever possible.

Oh and chill in rafters for an extra minute when there's one guy left in a stealth room just to watch him scurry around in terror for a bit. Once you decide to actually progress land behind him (again don't cancel the animation) walk up to him, wait for him to randomly turn around then punch him in the face.

The gun jammer is your best friend at this point.
Oh haha that's what I usually do, and if there's one guy left I always knock him to the ground, then go hide somewhere else, so he's terrified :D

I play all games through on the hardest setting first to ramp up the challenge and i refuse to use walkthroughs until i've finished the game once.

Other than that though i've done a play through of GTA4 without getting busted or hospitalised which was a nightmare and suprisingly easy in equal measure. Also tried playing the entire game without using a taxi to take me to a mission but eventually it's just too annoying.

I played fallout 3 through without buying any weapons or ammo from the shops. I also only carried 3 weapons and no more than 100 ammo for each of the weapons. Suprisingly easy if you use the right weapons, even the deathclaws

In Medieval: Total War II its not uncommon for me to attack vastly larger armies just to try and get some difficulty. I've never really been beaten when I was the aggressor. The longest odds was something like four to one and I annihilated the enemy. Feels good man.

It was actually a straight-forward boss battle but, finally beating that douchebag in the white suit at the end of Onimusha: Dawn of Dreams. Screw that guy.
Capthca: Apparently, "only from Dicks" is not a suitable answer for dish network

To put it into perspective, Those 10 auto-revive potions you get on Easy Mode? I used the last one at the end of the damn Strip Club. That's not even halfway through the game!

My hardest had to be Persona 3 on Maniac. Oh god that was horrible... never again.
