Moto GP 2. That game was unforgiving.(shudders) Warhammer Dark Omen. Actually got to the final level, but that boss is a cheating {colorful b-word}. Three Dirty Dwarves for the saturn on hard. Oye, it was murderous!

I forget what it was called, but I remember a non-turn based game that had a tutorial on a difficulty I could have fun on, and then the missions involved me getting pwned, on easy. It was maybe called Empires at War, or Civilization at War, and wouldn't have been any later then 2002, 2003 at the latest. I remember being the German's fighting the allies, controlling the Red Baron while against allied ships on one mission. Another had me as futuristic Chinese fighting some rebel groups, who nuked my gate about a minute into the game, and then sent in the ground forces to obliterate me. First mission. Another was a futuristic mission where I had to sneak out some leader of a Russian rebel group or something. When I tried to go straight out, they checked in the trunk and found, and I had to sneak him out ina carrot truck, I think. ONce I got him out, I had to build a base, but was destroyed by their huge forces. It went from historical battles to futuristic. It was fun, but way to hard, so I gave up on it.
Pac Man. I've never got further than level 8. The original arcade version has no last level. Plus, the 256th one looks like this:
Cabbage Patch Kids for the Coleco Vision! I've never seen a children's game more unforgiving than that, coupled with those awkward controllers.

I would like to add a couple I just remembered - GUN.SMOKE from the arcades, especially the PSP port on one of the Capcom Classics Collections. It was about impossible on an arcade screen; on the teensy-tiny PSP one you can hardly see the bullets at all, and the room to move out of the way in is equally shrunk - far, far too tiny. The scarceness of checkpoints doesn't help either, nor do the enemies that spawn BEHIND you since you can only shoot forwards.
(The same collection also has ports of Ghosts n' Goblins and Ghouls n' Ghosts - both insanely difficult but actually beatable with some perseverance. I doubt GUN.SMOKE is in that version.)
Also, the last set of levels/movie (James Bond style) of the first Stuntman game ranks very highly on my impossible list. I actually came away feeling vaguely violated after trying a hundred times or so to pass the third level of five, a feat no other game has managed.

I had more than a bit of a hard time with the NES version of Ninja Gaiden and it's sequels, plus all of those games with no end. My best for Tetris is 26. Final Fantasy and Dragon Warrior wern't hard, just time consuming.
Toy Story for the SNES.

As far as old games go:
Battletoads - I couldn't even beat the game with an infinite lives cheat!
Super Ghouls and Ghosts - Just too difficult in every way.
The first two Oddworld games - Very difficult but still a lot of fun.
