All the schools in GTA:SA with gold medals, that was the most frustrating part of getting 100% completion in the game. And afterwards I learned that this is not even necessary to get 100%.
Especially that sprint circuit where you're nudging the end cones closer to one another to shave distance off the track and a spare millisecond off your time...

Beating all the cheat times in Goldeneye - except Facility 2:05 I've seen a video of someone doing it. Pah! Looks like machina.

Ahh, Timesplitters 2 on hard. Man were there some really difficult bits on that game. I just remember NeoTokyo being so hard, having to follow the woman to the base without her or anyone else spotting you.

Me and my friend had to do a similar thing and map everything out, down to every camera movement and every time that stupid woman would look back ver her damn shoulder :\
I'll second this, but i've done it. I bypassed the follow, by dropping down the street and waiting for her in the sewer, though.
Some of the Timesplitters 2 Arcade and Challenge Platinums were well tough. So i'd probably put that on my list. Haven't done them all though...

On Timesplitters: Future Perfect I couldn't get past the massive Splitter monster in the church - I know if I beat him i'd clear the rest of the game. Irritating. Maybe I'll revisit it.
Man some of those challenges were damn hard... It was such a good game though, and so was Future Perfect. But that Splitter Monster was pretty hard to beat too.

That last boss on Final Fantasy X, I had to go completely all out to defeat it(Use all summons and items) you finally think you've killed it and then IT GETS BACK UP WITH DOUBLE ITS ORIGINAL HEALTH. Anyways I didn't play it ever again after that.
It wasnt that hard.. i got it killed after few tries.

I was like 12 at the time... I should probably try again someday (I did the exact same thing with FF9 the end boss was too hard so I just left it, was too lazy to grind more levels)

Incinerating the Faithful Companion Cube... I'm still unable to sleep at night.
But seriously, I think it would have to be beating the Nihilanth at the end of Half-Life. I hated the gravity situation.
What he said. Getting teleported half the time really pissed me off as well.

It's difficult for me to say with video games. The single most frustrating experience that I eventually overcame was given to me courtesy of Ghost Recon (the original). The game shipped with about 50 different challenges to complete. One of them had you attempting to complete the last level of the game (Red Squre, a fairly close quarters map for the game), on Recon (a mode that required you visit a number of spots on the map in order to complete it, killing people was purely optional) on elite (meaning there were 50 defenders), using the M40 (a bolt action sniper rifle with 75 rounds) and grenades (which were damn near useless).

Anything but a clean headshot rarely resulted in a kill, every shot required the same length of reload as fully reloading the weapon, the route forced you to fight everyone in the map and you were almost always facing rushes of 4 or more people, without a lot of range to work with (perhaps a city block at best). To make matters worse, at the time I was playing the game on a 7 inch LCD screen (I was deployed at the time and the screen was small enough to actually bring with me). Anything other than near perfection would result in either your death or running out of ammunition before completing the map.

Achievement: Skyjacker in Halo 3.

Map: Valhalla.

Situation: At home base, enemy Banshee approaching, step into Mancannon... weeeee! Oh, look I happen by chance to be flying past the wing. Hold button to board in mid-air... unexpected success... so stunned by what I have just done that I crash into the waterfall. Death ensues.

That was pretty frustrating, but I found City Slicking to be even more frustrating, with that oversteering piece of shit.

For me? Probably has to be not chucking the 360 and Halo 3 out the window after hearing my younger brother screaming at it for six hours straight.

Killing the whole counter-terrorist team (12 people) in Counterstrike Source on my own. In a matter of seconds my whole team was massacred and I was the only one left (probably because I always linger around in the back). Was so epic, the first thing I thought of afterwards was the manhunt in the forest from Rambo: First Blood. I kept moving around, sneaky knifed some of em, sniped some of em, and went LEEEEROOOOOY against some of em untill they all were dead. It was truly epic, after the match all I did was shiver with an :O face thinking "How the HELL did I do that?!" Funny thing is that everyone betted against me, so everyone lost all there money unable to buy proper weapons :D A mix of pure awe and slight anger, I immidiatly stopped playing, you gotta stop at your highest point right ;)

The hardest difficulty on Far Cry 2. Every encounter was dreaded, in the end I had to drop it by a level. The difference was massive.
Infamous isn't too bad, but you will definitely learn to fear those road patrols... so much lost progress due to the fact that they can often kill you faster than you can hide somewhere. Stealth is basically the only option.

One of the hardest things I've done recently is completing the Flopside Pit of 100 Trials from Super Paper Mario. The first one is pretty tough, but unless you level grind like crazy the second one requires a minimum investment of at least an hour, and there is a very good chance you will only make it to floor 80 or so. I died twice on the final boss before completing it, and it took me a good six or seven tries... not the hardest thing ever, but incredibly frustrating due to the repetitive nature of the task.

Mass Effect is also ridiculously hard in some places if you play on the Insanity difficulty, especially with certain character classes/party types. There are some enemies in particular, like Krogan, that not only have massive amounts of health, but also are able to become invincible for several seconds, regain all their hit points upon death every 60 seconds or so (meaning you have to kill them a minimum of two times), and can kill you with one or two melee strikes and have a nasty habit of charging you. There's two parts especially, the first on Therum and the second on Vermire, where due to either their numbers or environmental conditions, I had to fight them a good 20 times over just to win... oh, and don't forget the unskippable cutscenes that happen before those fights. Thanks, BioWare.
