Real Life: As I am Australian I have not physically seen many weapons in general, the most exotic are curved 'display' blades with beautiful handle work and often many well crafted blades. The stuff Kit Ray does comes to mind, of which I have seen and even held a few of his creations. If we go to pictures of weapons, well there are so many good sci-fi and fantasy drawers who can create things that couldn't really exist in reality so I assume you mean physically seen.
In game: Anything biologically created has to be the most exotic weapons I have seen. Sadly it is rare for these to be done right, most of the time they are just a conventional weapon skinned to look a little 'freaky.'

I kinda like the idea of the Soul Cube from Doom 3. A sentient, Pandora's Box filled with assorted sharp objects, controlled telekinetically.
