I think Shenmue is pretty unique. Apart from the sequel obviously i have never played a game like it.

Hmmm... hard decision because I'm kind of a connoisseur in case of original and unique games.

My personal favorite would have to be X3 which while not being the only "space sandbox" game (there is freelancer and... and something else I'm sure) it's the only so VAST and perfectly designed, You can do whatever you like build your stations trade for the entire game end never even ONCE shoot a blaster, or you can totally ignore trading aspect and just become a merciless pirate. You can do whatever you like and that's why I love this game.

other game of the "unique" type would have to be Daggerfall. Yes I know that it's in fact "Buggerfall" because of the amount of glitches and bugs this game has but it is so fun! When you realize that the game world is HUGE and you can do anything you want it gets even better. While in the TES series I personally like Morrowind the most (oblivion is just... empty) Dagerfall has a special warm spot in my heart for being foolishly brave and exponentially fun.

Third game I will mention is Mount & Blade for its pure fun factor. And as far as I know it's the only real "knife simulator".

Last (but not least) of the games I will mention (since there are soo many more) is Hearts of Iron...
Yeah I know that's unexpected but there is something enduring in just building your country for world domination in a way that is not friggin easy :).

As said these are not the only unique games in the world, hell there are many more like Psi-ops, Scrapland, Startopia, Sniper Elite, Thief and Dungeon Siege.
But those mentioned are closest and dearest to my soft gamer heart :).

Ico. More so than Shadow of the Colossus, I think...or possibly that was just because I played Ico first, and so went into SotC knowing what Team Ico was capable of.

Burnout Revenge. It's not quite a racer and it's not quite Destruction Derby either. Only car-based game I've actually liked.
One of my favorites, one of the reasons I got a ps2 was for Burnout 3. Too bad the franchise has gone downhill from there. What other driving games reward you for causing the largest crash possible. They could make a whole game of crash junctions and I would buy it. In fact they should do that just to make it up to us for making their last couple of Burnouts completely lame.

The other reason was to play Katamari Damacy which is probably the most unique game I've ever played... next to Loco Roco.

Valkyria Chronicles is also very unique and possibly the best way to bring strategy games to the console. It's kind of an srpg but it has shooter elements to it and a great style. Never played anything like it.

Then there are games that look unique but are just re-skinned versions of common games: Ribbit King is just trippy golf and Mr Mosquito is just trippy flight sim. Sims, a trippy dollhouse.

The most unique game for me was already meantioned, so I will try to give you a different angle on things. If you want something entirely fresh, you might want to try something really old: textadventures. That's something you won't find among regular games or even gaming sites, but there is a functioning community behind this and they are mostly free.
It might be the refreshment you seek.

On the other hand... "The Path"

Of the top of my head; Eufloria. Incredibly slowpaced, this is a RTS that has no resources and encourages Zergings.

Star Control 2. Still haven't come across a game that did it so well since almost 20 years back.

The first 4-5 games I played, as well as the first few games of every genre I got to play.

Uniqueness and originality are utterly subjective.

I'd say Heavy Rain. It was unique as it drew me in, in a way no other game has. It had a story that I loved (not uncommon in games for me but that coupled with) characters that I wanted to craft in a specific way. I wanted to shape the story myself and loved getting to the end. Also it is the only game that has managed to make me angry at what happened (not just angry about losing but angry about killing off a character I like by the fact that I screwed up), struck by indecision, tense/stressed and sad by some of the things. Plus I felt that the gameplay was really well done for the game of that kind. Yes...I love Heavy Rain.

Condemnded Criminal Origins was great, it restored my faith in good video games for the moment, and who knew, IT WAS MADE BY SEGA. IMO the only good game thhey have ever released.

The Void was unique. So was Spyro the dragon. Granted I haven't played that game since the ps1 era.

Perhaps something like Dungeon Keeper, yes that was pretty unique. Well, except there's DK2 which is almost the same, so there's two games with the same mechanics. That kind of makes it less unique. There's also Dwarf Fortress, which reminds me of DK a lot.
