I think that the scariest monster... TOM NOOK!!! He's not a "Monster," per say, but he is the God damned most creepy character and/or villain.
ReDeads from Ocarina of Time were the first monsters to freak me out. I literally had nightmares.
I was also pretty scared by games with giant fish that could come along and kill you instantly while you were doing something underwater (Mario 64, Banjo-Kazooie...). Still gotta give the prize to ReDeads though.

Regenerators from RE4, freaked the shit out of me when I was like 13 and I REFUSED to go in the freezer room where there were a bunch of them hanging up on meat hooks. Ironically that is where you get the scope that makes them easy to kill, so they were always a ***** to kill when they shouldn't have been.

the flood on halo scared the shit out of me when i was younger. i didn't finish the game past the first flood level till i was a couple of years older cos i was too scared to play it.
those splicers in bioshock that prented to be statues were pretty scary. i was grabbing ammo off the floor and when i looked up one was right in front of me. but the one that had me shitting my pants most (not counting the flood) is probably the first beserker on GoW 1. I was disappointed they weren't in the second game, i thought they were awesomely scary.
