Underwater levels are the main offenders for me. Also I hate sewer levels, there's nothing that will make me turn off my game console faster than a sewer level.

any level, or game for that matter, that doesn't portray the US military correctly. Soldiers getting shot left and right, no one knows how to use their weapon, no one is moving in squads or fireteams, no one knows how to lay down suppressing fire or call for air support/medevac, no one is in fireteams/squads and watching sectors etc..etc..
it bugs the shit out of me.

Anything with low lighting, hard navigation, and enemies that pop out.
"Movement, SHOOOOT. Alright now lets go... um well we've been going in circles, bust out the map. Hmmm well..."Enemy screams and jumps you" Heart Attack.
I gave up on Bioshock, and forced myself through parts of Half Life 2. Pop out things scare the crap out of me, and being tense and nervous about the shadows is'nt much fun to play.

Water and Timed missions occasionally make me almost as frustrated enough to throw the controller/keyboard/mouse but escort levels make me smash the screen with what I'm holding.

There should ONLY be escort missions if you can control where the thing your escorting mindlessly dawdles to.

Escort missions, too. I think Half-Life is the only game who did escort missions right: You're escorts cannot die (unless they get three shots from a shotgun at point-blank), they help you with combat, you know the route, and you only have to worry about yourself.

Water levels. I fully agree. I still remember that level from Banjo-Kazooie.

Racing levels. I bought a fighting game so I could beat people up, not so I could race.

Defending levels. I hate it because usually you're overwhelmed., and all assistance is useless.

Capture the Flag. Getting to the enemy base, finding the right room, going out, and going to Point X without dying? Really?

Timed missions. I hate the stress.

Chase levels. I do suprisingly well on them, but I still hate them.

Horror levels. I hate them because it mainly includes the hateful pop-out scares.
So you hate every kind of level except go there to win style?

Almost anything with a time limit.
Why the fuck do devs still put timers into missions?

It's clearly obvious that gamers hate them.

Assasin's Creed 2 was my game of the year but I almost broke the disc because of the timed race missions and escort missions.

Oh... and God of War: Spinning blade columns. Nuff said

Side scrolling that move the camera for you and if you go too far ahead or fall too far back you die.

Protect missions, especially the one in Nation Red. If the damn zombie runs into the guy you are dead, they don't even have to hit him.

That is about all I can think of right now.

Levels where you're stuck in a "supposed to die" scenario and they don't tell you. Like the monster that kills you after the tutorial of Demon's Souls.

Actually, I hate a lot of introductory/tutorial levels. So many games fail to make them blend into the game play, and a good number are also too long. When it is the one part that has to be pretty much the same every time you start a new game, you want to keep it brief.
Shadowrun for the Xbox 360 had one of the longest and most boring tutorials. Doing all 5 tutorials in a row without failing once would still take about 40 minutes to complete. Since the game was pretty complex you couldn't just jump right into the online mode either.
I...I am shocked! Isn't that the FPS one? How much tutorial do you need for a FPS???? (Look over at new Left 4 Dead 2
)
Yes, yes it is. Well, apparently there's a lot of magic spells and techniques that the game loves to tell you very slowly and with long intervals in between everything. Also, Left 4 Dead 2 doesn't have tutorials if that's what you're implying.

Convoy missions where you have to use a mounted gun and there is no option to dismount. Oh and don't forget escort missions.

TIMER LEVELS UNDERWATER!!!! and yes there is one.

But for me it absolutely has to be the level in Frogger where everything is dark and you have to collect these lightning bugs that dont last forever while guessing your way through DODGING ENEMIES.

Also that level in crash bandicoot, i think it's called Lights Out
