I'm Trypanophobic, scared out of my mind by medical procedures that use injections/hypodermic needles. Absolutely terrified by it, I can't even think about getting an injection or seeing someone else get an injection.

Very small spaces. It's all good until something goes wrong like something moving inside the small enclosed tunnel.
To be honest, nothing at all really, at least that I can think of. I used to be afraid of heights like all rational people, but they don't even bother me anymore. The inevitability of death no longer scares me either, I mean if a guy pointed a gun at my head I'd probably be scared shitless, but the realization that I'm going to die someday doesn't bother me. Plus, many things creep me out such as bees, wasps, and spiders but I'm not truly afraid of them, just wish the bastards would stop flying into my ear and making that damn buzzing sound. Weirds me out when that happens.
That my life has and will be meaningless. And most likely it will, so nothing else truely terrifies me.
What scares me the most is the inside of pipes that are large enough to contain a human being. Those pipes hold the promise of drowning (not always in water!) in cramped darkness or being torn to shreds by spinning turbines in cramped darkness. Positively horrifying. 
Spiders, dogs, the undiscovered sea, myself at times. *looks into mirror* AAAGGGHHH!!!
