In the first game, I used Liara and Wrex/Garrus pretty much exclusively, depending on whether or not there was anything that needed to be hacked. I took Liara and Garrus to Ilos, and they are going to be my default team in the third game. It just seems right, my woman on my left and my homeboy on my right :).

In ME2, though, it was usually Garrus and Mordin, though it would change depending on which characters made the most sense for the mission. For instance, Tali came along on Legion's loyalty mission, since we were killing geth and she would enjoy that. Also, I brought Tali and Garrus with me in the final battle, since they had been with me since the beginning. My Shepard trusted them the most, and wouldn't want to fight a giant Reaper fetus alongside anyone else.

Basically, I like to play the game in a way that "meshes" with the plot.

I'm playing through Insanity right now, and I'm finding great success in using Grunt and Garrus. Its my second playthrough, so even though my shepard is a soldier, he knows warp. Plus when you get to Inferno Ammo, you pretty much annihilate anything, plus Grunt's fortification skill means i rarely ever have to use medigel on him. and Garrus and I are crackshot snipers.

Except Jacob. I despite Jacob just because I always play a girl and every single time I ever go to talk to him semi-casually he always reacts to me as if I just tried to hump his leg or something similarly distasteful. His disgust with my small talk makes me dislike him I guess - especially since I've never even tried to 'romance' him.
I'm so glad I'm not alone in hating on Jacob. I saw a YouTube vid of what happens if you break it off with him and tell him you're interested in Garrus. He calls Garrus a cuttlefish, which sounds like a xenophobic slur, so I hate him on principle.
