Playing Mego-lo-mania on the megadrive and seeing a section on the map has been nuked. Specially since when you first get nukes there is no way to defend against them and you have to pray that you can get enough people to build new places on the map and assault the enemy.

Yikes. Those kinds of things always unnerved me since "Tremors". Same god damned thing in Jedi Outcast: Jedi Academy, where you go looking for survivors on some desert planet and then have to salvage pieces of the wreck while be hunted by those giant worms. Really makes you twitchy.
Yeah, that was the first thing that came to my mind whent hat happened. =S

The part in Bioshock, where you go down into an almost pitch black, water logged basement to find a Power to the People, and there is a paper-mache/staue/person/thing-y, sitting in a chair facing the corner. See them everywhere so I payed them no mind. So you go and use the PttP and turn around, and that statue is in a balerina pose right behind you. made me jump because I wasn't expecting it. I get rather trigger happy when I'm surprised, and forgot to change the electirc buck to normal, in the water logged basement...

Fallout 3, every time I'd walk through a door, a super mutant or ghoul would pop out of know where scaring the hell out of me.

That was a 0.o moment indeed, and I'd even been forewarned by a friend about it.
Kind of against the spirit of this thread, but certainly the most soul-searing experience was when I couldn't log into my WoW account and thought it had been hacked - turns out a friend had changed the keyboard language as a joke :/

I think these two are both fine examples of disturbing constituents in, otherwise, more or less scare-free games.

There's also that brief segment in Blade Runner, after your investigation at Howie Lee's Sushi Bar led to...

Most people wouldn't agree with me on that one, and I know it's a rather pathetic thing to be startled by when you take a step back and watch it passively occur on-screen.
...but when you've played for a few hours and allowed yourself to be completely and fully immersed in that minutely designed world the game (and to a larger extent, film) presents, it takes on a whole different character.
One of the great P-A-C's.

Mine was Turok, my friend was watching and a velociraptor jumped us. We almost pissed ourselves.

EDIT: Oh I forget my other batshit scared moment, it was Okami. when the water dragon jumps out of the ocean RIGHT behind you.

Does Fallout 3 count as horror?
I jump every single time that I kicked a kicked a can or bottle in the hotel where you need to repair the elevator. I had already killed every enemy in the building, so I figured there was some scary thing running around with me in there.
The isolation really gets to me.

The music is evil and... The boss has no sprite in the second stage. And the attack messages make no sense.
Who the hell could come up with something this evil?

One time when i was really bad at Gears 2 me and my cousin were playing horde and i wasn't paying attention to the radar or the thing that tells you how many enemies are left, and i said "Well I think that's all of them" and all of a sudden I heard "Hunger!" and then my guy just died a split second after. It scared the crap out of me

Just today. I was playing BF:BC and I'm taking a control point. Then the little YOU ARE LAST MAN thing pops up I take cover on the porch I'm near. Well turn out my squad spawn right be hind me. Like two inchs away. So when I check my flank. I see this guy, uzi out and next to my face, I nearly jump out of my skin and almost mow him down before I see he's on my side
