When I was like 9 or so, I was playing WWF Attitude on the PSone. One of the wrestlers was on the ground for a long time, and when he got up he was all glitched out and it looked like we was melting. Then he did a weird jump, disappeared under the ring, and the game froze up.
Actually a lot of games on the PSone scared me when I was like 7 and 8, mostly because everything 20 feet in front of you was black because it wouldn't load. Made everything a little creepier.

In Batman: Arkham Asylum, the patient visitors room. The way it forces you into a first person perspective makes it feel really claustraphobic, and the Joker dummy is pretty creepy on it's own. Then you look away, look back and JESUS CHRIST DID THAT ARM MOVE? Okay, my reaction wasn't that extreme but it's still a pretty creepy room.
Another scary moment is in Mirror's Edge when Rope Burn lunges at you from around the corner. That really suprised me the first time.

I was playing Half-Life 2 on the Water Hazard level and I was investigating Station 7; I was climbing around in the barn when I stood an unbalanced plank, launching a headcrab straight at me!
It scared the life out of me.... Not to mention the lights were off....

The moment when you're in the morgue and you exit the room after the scary voices tell you too and you're RIGHT BACK IN THE SAME ROOM. Freaked my rightly the fuck out more than it should have.
I know you'll find this silly, but I'm terrified of Azure Kite from dothack//GU. I had a dream where he was haunting me in a public bathroom. Also, in dothackGU vol. 2, you see the PC ghost of Phyllo, and he's really creepy cuz he just floats there and stares into space with eyes so wide he must not have lids!
