Mutant League Hockey for genesis, it's kinda goofy by today's standards, but back then I thought it was the most violent thing I'd ever seen outside of a movie.
I can't quite remember which one was my first real violent game, but it was either Killer Instinct or Splatter house. But hey they were both great games as a matter of fact I still play Killer Instinct.
Grand Theft Auto 3. Mind = Blown. I don't usually go for violent games (less than 2% of my collection is M-rated) but the GTA series is just so well-done I have to admire it.
Star Wars: Dark Forces. It eventually became Jedi knight and jedi academy and all that stuff (the enemies weren't even 3d). My dad took it away because he said it was too violent and he said he'd give it back when I turned 10. I remembered, asked, and he said he threw it away.
