1) Cluster Bomb (Rainbow Six Vegas 2) - Kill 3 enemies at once using explosives with at least 6 players present

2) Completionist (Mass Effect) - Complete the majority of the game

3) Need For Speed Pro Street - beating all of Ryo's records

I'd probably say the same as LethalNiMiTz, and would also like to add, all achievements in COD4.
That, Zombie Chopper, and One Free Bullet. Surprisingly, my gamer score is only ~5300. 'Spose it's because of my small library (10 games, 2 of which are arcade games).

One Free Bullet, Little Rocket Man, and getting all of the Bioshock Achivements. Seriously, I 100% the game. No, I don't have a life.

Spartan Officer feels pretty good too.

The "heal a shit-ton of people as a medic" and "have an enemy medic heal you as a disguised spy" were pretty fun in TF2, or maybe "Become the Archmage" in Oblivion was pretty fun as well.
// note: I don't remember their names so I had to paraphrase what's needed to achieve them.

I think the "beat CoD2 on Vetran" was the most rewarding Achievment I have ever gotten. That game was very hard.

...I feel like an achievment whore actually talking about it... getting all 1000 in DMC4 (Specifically, getting S rank on everything). Also, full 1000 on Eternal Sonata, as well as Assassin's Creed. Really, getting full on any game is always a surprise for me, 'cause I never actually go for them. I just play the game, and I happen to be a completionist.

Seriously though, I don't actually pay attention to my achievements or Gamer Points. I suppose there's an H3 one I'm proud of, somewhere. "Fear the Pink Mist"? Maybe.

'Seriously...' - "Kill 10'000 people in online ranked matches" - Gears Of War. Since I had to get 11'380 kills before it gave it to me it was immensely satisfying.

I haven't got it yet, but the day I get the achievement for winning the campaign on veteran on CoD 3 is the day I finally become full and happy...

*Sigh*... Someday... Someday...

But seriously, that is one hard ass achievement to get...

Now, one that I have... 'Overkill' on Halo 3... Kill 4 people within 4 seconds of eachother on a ranked free for all playlist... *Shudder*

I can't think of a particular ninja achievement right now. At the moment I'm more concerned with my score ratio (ratio of available points to earned points for all my games.) I want it to at least 50%, and need about 1200 points to do it.

Complete CoD2 on Veteran - My game was broken, and I had to play through the russian campaign over and over again, as every time I finished it instead of going to the British campaign, it restarted the game...

Complete CoD3 on Veteran - I'll never forget that forest...

Complete CoD4 on Veteran/Mile High Club/1000 points

And apart from the CoD games, which seems to be the only game I'm half decent at, I'd say 'The One Free Bullet' for HL2E1 was good to get. And the acheivements for completing the Guilds in Oblivion were nice too.

Base Jumper from Crackdown is up there for me. Totally off-the-wall and fun to do, too; plus the view from the tower was amazing.

Completing Halo 3's campaign on Legendary was also deeply satisifying.

I found the singing Vortigaunt without assistance, and cleared out the lambda caches.

Oh, and completing Marathon: Durandal.
