I'd have to say some of the hardest games I have played are the very original Rayman and Ninja Gaiden 1, neither of those games are easy at all.

Ghosts and Goblins. I've only played it a few times at my cousin's house, but it's pretty freaking hard. Unless you get the lance, then it gets semi-hard. Either that of Men of War, probably because I'm a perfectionist and if I lose my most valuable unit I have to restart, but then the AI somehow knows where my guys are... Also, IWBTG is not really a game, it's more of a "how many times can you do this trial and error exorcise before you destroy your computer" type of thing.

IMO, I Wanna Be The Guy doesn't count, the whole game depends on memorizing where the death traps are, that's trial and error gameplay, not real difficulty.
Edit: Son of a *****, ninja'd.

Jim Power in Mutant Planet for the Atari ST, it was hard as hell. The one thing that kept me going was the desire to hear all the music. Just listen to the song that plays on the first level.

Ninja Gaiden 2, easily. Thought the idea of having a difficult game to keep me entertained was a good one. It was just frustrating. Add to that that the idea of Ryu being a super ninja is laughable, and well, not a fun game.

For me it would probably have to be Battlecruiser 3000AD. To be fair, a lot of the difficulty lies in the impenetrable UI.

Probably Tohou 8...

That, or Dragon Age: Origins.

Though granted, I was playing on the middle difficulty out of choice, but it's still sometimes difficult of easy even.

Bullet Hell: Touhou games. Probably Touhou 11 (Subterranean Animism). SO MUCH BULLET.

RPG: ADOM (Ancient Domains of Mystery). You've heard of Nethack, right? Or Rogue? It's like that, but makes them look kind. Finished it, though.

Platformer: I Wanna Be The Guy, predictably. Finished it, but ow. Ow ow ow.

FPS: My ten minutes or so of playing some mission in Call of Duty 4. All I ever played of a CoD game, and... well, I'm no good at any FPS in general, but that appears to go double for "realistic" ones where you die from a stiff breeze and harsh language.

To be honest, Dragon Age only seems difficult at the beginning. Once you get a feel for how everything works, most of the game becomes much easier.

I felt the same way about The Darkness. Not being able to move at more than a brisk walk kinda made me have to restrategize.

To be honest?

The first Harry Potter game. When I was little, I couldn't for the love of all that is holy beat Voldemort.

I could later, but back then, man it was hard.
