Silent Hill 2, I know it's a cliché to say this, but this game deserves it. First time I played it, I couldn't get past the first time a monster you show up and you go back to town. I mean the radio static, and just turning around and around waiting for a monster to show up out of the thick fog, I couldn't take it.

Also, this game has one of my most favorite video game moments. When you see Pyramid Head the first time, when he stands behind those bars with a red aura surrounding him, not moving. It's the only time a game has sent shivers down my spine.

FEAR + expansion packs being pretty much the only horror game that I've played, well... There's your answer :p

The first FEAR was startling but once you played it it was... 'oh... bad guy pops up here I remember' ... and lost alot of it's value.

The first Aliens vs Predator as the marine was creepy, I could never see shite with the normal motion sensor on ever. And so I ran through all the levels with night vision wich is really annoying when you hear the screeches of an alien comming from SOMEWHERE...
Also the level sent aliens at you from the end of the level so you couldn't just sit and go afk. Something WOULD be coming all the time.

And that's why i said parts of RE4 were scary. Such as the parts with those with Regenerators and Iron Maidens. Fuck. I especially was scared of the regenerators in the hospital area. Fucking hospitals . . .

Now the Iron Maidens only startled me but my friend saw me playing when I encountered one and oh boy. It scared the shit out of him, It was hilarious.
Yeah, that regenerator in the hospital was scary. Just knowing it was gonna wake up and corner you...

Dead Space, specifically when I learned Stasis and I cast it on Jumper who was trying to rape my face. Terror in slow motion! o__O

Doom series... when I was 5 Doom 2 was the scariest game. It was so bad I was having nightmares over it. I belived the pixelated monsters were coming to get me. Apart from that no game has scared me...

the first silent hill- Not only because I was about 10 when it came out- but back then I had a chipped ps1 and it had to be played with the disc tray up and a pen suck in the laser (Those of you old enough to have had a ps1 will know what i mean) So whenever a monster appeared, as well the radio static I also got my ps1 churning itself like a broken washing machine, sometimes acctually shaking with the effott.

Wow, I think I got scared by most of the games mentioned here. That either means I'm a pussy or I really get immersed when I'm playing. I'm going to go with the latter.
FEAR to me was more than jump scares. The first one at least. When the flashlight and HUD started flickering and I saw shadows coming from something behind me it didn't make me jump up. It wasn't that kind of cheap scare. It actually made the hair on the back of my neck stand up straight and sometimes the effect lasted for a while, so I would definitely count it as a good scare.

But I think the scariest one for me was Clive Barker's Undying. It might not scare you much now since the graphics are outdated and break the immersion when you're used to today's standards, but it really messed me up. Especially in the catacombs where you could hear crying in the far distance. It was the only game where I showed it to friends and they actually got freaked out by watching alone. You had an ability that allowed you to see things that happened in the past at the particular place you were at and you knew when to look because there was a whisper that would say "scry" or "look around". The whisper alone made me piss my pants for two reasons: (a) it was a really scary, soft whisper that you weren't quite sure if it was actually coming from the game or if you were hearing voices and (b) you really didn't want to see some of the things the ability showed you some times!

Admittedly, it was a very gory game and maybe sort of cheap. The story wasn't exactly amazing and the end boss was a joke. But the scares alone were worth it. I highly recommend it if you don't mind dated graphics.

I'm not a hugely scared person when playing games, it's more fear that if I use my "kill everything" button, then another everything will pop up straight after.
That said, I haven't played many games that are supposed to be scary, not out of fear, but out of lack of interest, or there being more interesting games coming out anyway.
If you ignore that, the scariest game I have played was probably Doom 3, if you wan't to know how scared, something I actually said while playing it was "MAKE THE HEADS GO AWAY!" Though that may have been more out of annoyance of being clusterfucked by the flying heads and having no shotgun ammo.

bioshock has scared the crap out of me...doom 3 had some crap my pants moments....

really any game played at roughly 2am with no lights and headphones on (so i dont wake anyone up) scares me

i dont care if im playing "happy rainbow island", under those conditions my nerves get all jacked up and i freak easily

I would have to say that, next to system shock 2, Arx Fatalis was probably the scariest game I have ever played. Think Morrowind, except underground... and replace cliff racers with disgusting rat-men.
