XCOM, any time I send my soldier to a spot behind a car just for the full force of the enemy troupe to blow it up in one turn... INSTAKILL. Or Black Ops 2, when
you shoot the dude in a bag
and immediately after I was like 'oooh I have a bad feeling about this'.
What can I say, he was the protagonist in one of my favourite FPS games ever. I felt bad.

I accidentally shouted at some NPCs once. By shouted, I mean Skyrim shouting. I only had the first word for Storm Call and I had it as my active shout. I was happily going about selling things in Whiterun, and then I put my controller down. Since the PS3 has no guards on the triggers, my character said the shout, and meteors rained from the sky at all the innocent villagers. People started chasing me while I was saying "I didn't mean to!".

I reloaded, but I still felt bad.
There is something about Fallout/Elder Scroll games which lends itself to remarkable carnage. I'm fairly certain in every iteration I've been caught do something minor like pickpocketing or jaywalking and it has escalated to the point where I slaughter the entire town.

There's been a bunch of horrifying game moments that were caused by my stupidity (and lolsiness) but yesterday I was playing Farcry 3 and decided to swim through a river even though it was swampy.

SUDDENLY, CROCODILE

That was scary, out of nowhere, QTE's got me there even though I survived... And then I was out in the water trying to kill sharks, and I fell in and saw two sharks nearby and I was panicking in my head because I hate open water and just thinking about it (let alone doing it in a game) is flipping scary. When I got to my boat and climbed into it I had a shiver go up my spine.

God, the same thing happened to me. I didn't know there were crocodiles! I was happily hunting some Tapirs with my bow, and one sprinted over the river to the other side, so I happily splashed in after it.

SUDDENLY, CROCODILE

I almost died in real life. Almost. I got to laugh at that moment later in a Two Best Friends episode where they also encountered one. They actually saw it swimming towards them, though, so it wasn't quite the same situation...

The sharks have also got to me a few times as well. I was hunting a relic and happily swimming up to the boat above me, and then I was in a shark's mouth with blood everywhere. I went something along the lines of, "NONONONONONONO!".

Captcha: need a bigger boat

That's freaking hilarious, Captcha. You should be a comedian with that wit. IT WASN'T FUNNY!

In my first playthrough of Mass Effect, I let the Council die aboard the Destiny Ascension. But in Shepard's defense, that Turian councilor is a dick.

I was watching my friend play Far Cry 3 for the first time. Already finished it so I knew that deep sea usually meant shark-with-a-mouthful-of-teeth.

My friend was swimming so he could use water as a shortcut (straight lines and all that). I looked up the Jaws theme on Youtube and started playing it. He laughed at me, thinking it was a joke. Then he went apeshit and dropped the controller because he was frightened by the huge maw that wrapped around Jason Brody's forearm at the exact moment the Jaws theme goes intense.

He needed a cig right after. :D

The only time something happened in a game that I didn't mean for it to happen (wasn't scripted, like Spec Ops) that caused me utter horror was in Mass Effect 2. As soon as the crew was kidnapped, I didn't go after them right away. I did a few more side missions. Needless to say, the moment I watched my favorite red head get turned into goo while she plead for me to save her, I reloaded a previous save. Lost several hours worth of gameplay, but I wasn't going to let that happen.

In Dark Souls I accidently hit the giant blacksmith. No big deal right? Go to a different area, come back and he'll reset right? Wrong :(

Joining in with the Dark Souls party, I killed the Undead Merchant because I thought he was an enemy. After learning from this mistake, I decided to hit the Female Undead Merchant because I thought my weapon couldn't hit through the bars. I was wrong. Luckily I was able to pay Oswald to make her forget that ever happened.

Fallout 3: the whole Tenpenny tower thing.
I got the ghouls in, accidentally letting them murder every last person, including Herbert "Daring" Dashwood. I got revenge for them and myself by accidentally bludgeoning every last ghoul to death with a hammer and then accidentally removing all of the limbs from their corpses.

I was playing Fallout New Vegas and went to Quarry Junction with a low leveled character.Biggest mistake ever as I died 23 times to deathclaws.
This same thing sort of happened to me in Dead Wind Cavern, but I had never played a fallout game before and so I had no idea what a deathclaw was, until it was too late ...

baldurs gate complete,

i decided in the town before the mine that it was a good idea to pickpocket some random npc i had a save quicksaved already so i just thought why not, so i did and it failed.

but then i noticed the npcs didnt go hostile to me, so i just thought... well they must have not noticed or something.

I....WAS .... WRONG

i went to resurrect a dead character. came out of the temple

and boom.

im just going to recount my characters last thoughts.

Well hello there a entire town filled with angry npcs, im just a casual level 2 peasant walking through, no i did steal anything, why is there a crossbow bolt firmly wedged in my anus.

or that time i accidentally triggered the expansion pack, why running arround randomly
i accidentally got teleported to a area after talking to a npc and agreeing to do a quest, where i couldn't escape from, and was told to find something *i forget what it was at this point* then surprise two mages coming to beat the living hell out of me.

and i didnt have a save this time so i literally had to re roll my character

Don't worry it's next to impossible to get them on your first go.
I've seen someone find them, then not know to defuse the bomb.

I was even worse, I hung around the office rummaging through people's desks rather than going straight to the mission. By the time I got there all the hostages had been executed. I had no idea this would happen. I felt very bad.

XCOM.

Was playing on Classic with a squad of one veteran sniper and the rest had varying levels of experience(everyone else was incapped from the last mission).

I was trying to position my vet to a high vantage point, but I dunno what happened next. Framerate drop, poor camera position, whatever the reason, my sniper ran right into a damn Chryssalid. Got egged, sent the rookiest of the rookies into panic mode, and they ran into some corner of the map that revealed a bunch of mutons, that panicked the 3rd squadmate when they fired on him. All I had left was one heavy. Then my panicked soldiers killed him...

I suppose that this is just another sad XCOM tale, but the fact that this happened because I made a mistake in positioning one guy...
