For me, it's introducing an interesting element, then making it near impossible to use for the next 3/4s of the game.

When a game tries to be realistic. It just makes the unrealistic moments all the more apparent and ruin the experience.

Escort missions.

I have an eternal burning hatred for them, especially when it turns out you never needed the character in the first place or they die anyway

NPC's that seem so intent to try to get themselves killed no matter how hard you try to protect them.

Whenever the Computer is a Cheating Bastard. Like how RTS computers can literally do multiple tasks at the same time. Or Smashbros. Screw you, Nintendog, block their vision for once!

Dissapearing bodies.Why can there only be three dead bodies on screen at a time, yet a game can have over twenty living enemies shooting at me? Do ragdolls physics really require more processing power than living enemies with movement animations and AI? Games that particulrly piss me off with this are Mass Effect 1 and 2 due to it being pathetically obvious. Bodies dissapeared as soon as you turn around or there are more than three. I still love those games, but jeez that annoys me.

Oh I hate escort missions with a passion, but another thing I absolutely loathe in video games is random encounters. This. Needs. To. Die.

Games that crowbar in online features and multiplayer modes that really aren't necessary. If multiplayer is the backbone of the game I'm fine with that. I'm talking about largely single player games that then shoehorn in online interaction because it seems to be the hip thing to do nowadays.

Being forced to keep weak NPC's alive through hostile situations. Usually this comes in the form of escorts, but it happens at random times as well. My companions in fallout 3, not to mention the damn caravans, getting shot to pieces within a matter of seconds was always obnoxious.

Why does that bother everyone? It's a shitty online mode and no one is forcing you to play it.
This actually does not bother me -too- much unless it gets itself a fair amount of advertisement. Napoleon: Total War was big on this because even if it seems decent in multiplayer, I can find so few good games there that its like the multiplayer element does not exist. But, I guess that's just a community problem in the end. Still, for all the time that those people spent talking up their new multiplayer features, it was very depressing to not see much ectivity with it.

Still, that's not a real game problem, which is why I didn't bring it up specifically in my first post.
