Escort missions.

I have an eternal burning hatred for them, especially when it turns out you never needed the character in the first place or they die anyway
I feel your pain. and by specially when they turn out I think you mean, they always turn out like that.

Crashing/glitches. If you release a game littered with bugs, it'll really upset the fans. Like..Fallout 3.
yea but i'm alright with all the little bugs that i find in Fallout 3, you have to remember just how fuggin much they put into the game.

Mute main characters, unless its a game when no one speaks or they speak just rarely.
I dont mind bugs and glitches, I can almost always install patches later.

My biggest pet peeve with modern games is that they are so shallow and mundane. They look pretty but thats it. Like a fake titted botox-lipped blonde bimbo.

As escort missions and QTEs have already been thoroughly and rightly vilified I'll go for invisible barriers/poor geometry.

I can see that enemy between the wall and the pipe so why can't I shoot them? And the number of times getting stuck on some bit of almost flat landscape or being unable to climb the hill because it's 1° over my threshold.

Those are what cause the most amount of swearing I do at the screen.

Tail missions... The ones where you "tail" some guy around without his knowledge...

They're always so slow, long, and boring... And alot of them make you start from the beginning if you're caught...

Invincible enemies that are only killable at a certain point or once you get to a certain destination. I am looking at you Grand Theft Auto, thanks for making me waste all my smg ammo on your stupid thief on a motorbike.

Mixed metaphors much? :D

Ot: It's been said but escort missions and timed missions. They need to die in a fire.

Pointless cutscenes.

Kingdom Hearts is a repeat offender.
You'll just finish watching a 10 minute freaking cutscene of another character telling you where to go / what to do next. It finally ends and the loading screen appears as you think "About time!". But what's this? ANOTHER cutscene! Is it some new plot twisting revelation? No, it's Sora turning to Donald and Goofy to say "Let's go!". AND THATS IT. You think "Are you fucking kidding me?" as you have to wait for the game to load once AGAIN.

Unnecessary.

Too many to list them all.

But my biggest one is when the game UI is just wrong (like in Space Empires 5, or Sword of the stars, or pretty much all game editors ever made) because its confusing, important info is hidden, and NOT user-friendly. Devs are lazy with their editors.. too bad since it makes the game last much longer.

When bad guys lose. (they should follow the evil overlord list)
I want my bad guys to lose because i bested them, not because they are dumb arrogant psychotic idiots and i brute-forced my way through 10000000 of his minions and yet he is still there waiting for me for a dramatic monologue followed by a 3-steps boss fight.

in-game Cutscenes where the hero is shown doing something inccredibly stupid, and there is noway to avoid it nomatter how obvious it is.

I agree, espicaly in the game Sacred 1/2, in both games, unless its a story quest they can die and they run into eneimies and never stop running, its really annoying.

I don't like games that rely to heavily on cinematic cut-scenes. Half Life 2 is an example of a wonderfully paced story that intrigues without breaking the action.

Quicktime events that are used in lieu of unique gameplay ideas. Metro 2033 has QTEs, but uses them with such a light touch I almost forgot it had them at all.

Terrible level design. FEAR I am looking at you.

Invisible boundaries in areas you could clearly access.

Games with vendors that don't allow you to buy back your stuff. I hate how I can sell a king's ransom to someone but their inventory never changes.

DLC designed to rip you off. By far and large, most DLC is made this way to take advantage of online distribution. (cost : content ratio is skewed wildly in favor of the publisher).
"3 weapons! Only 5 bucks!"

But we took away your right to mod the game so that we could sell you inferior content!

Or my favorite, the Madden 2011 "Steroid Injection" DLC (pay money to make your players better!)

Luck. I can't stand luck in games. Not just limited to videogames, just all games, full stop.

I'm a pretty chaotic person in reality, but I want my games to challenge me without rolling a dice. The amount of times I've lost entire armies in civ because 'woops, the dice roll came back sucky, you weren't using that stack of swordsmen, right?' I can't think of many other examples, beyond how in fallout, I can miss a 95% success chance 5 times in a row. Hate. Luck.

DLC. Escorts are annoying, awkward controls more so, but NOTHING is as annoying as paying 50 bucks for 35% of a game.

As much as I dislike playing alongside toal n00bs, it makes me really pissed when people start laughing into their mics about it and telling people to stop playing the game 'cause they suck so bad. They can't get better if they don't play folks! If you need to vent, mute your mic and talk to someone out of game about it.

This. Number one obstacle to new players getting good at a game is other people telling them they suck. My girlfriend stopped playing TF2 because of this, which irritates the shit out of me, because, somewhat unsurprisingly, I enjoyed playing with her.
