I suppose the thought counts, and thank the person that gave it to me.
Unless it was a passive-aggressive act to twist your nerves, in that case you do the same and plot a passive-aggressive payback.

If it's parents or grandparents, I just keep smiling until I reach the point where I can burn it.
Friends, though...Several of my friends have a habit of getting odd gifts for each other. One friend gave me a pink magnetic calculator and a tomato stress ball for my birthday. She got another of my friends a bubble wand that looked exaxtly like a *ahem* female sex toy. He responded by getting her a courgette. We just constantly try to one-up each other in the useless gifts department.

I tend to spend enough time with my friends and family to merit them having a decent understanding of the things I do and Don't care for. That being said I cannot remember the last gift I have receive that was "lame".

If people have trouble thinking of things that remind them of you, or that they remembered you liked...then you're doing a terrible job of being remarkable. Excel at SOMETHING, if nothing else excel at being YOU.

I usually respond with a polite, "Hey, thanks!" Because I'm genuinly touched that they gave me a gift at all. If it's incredibly lame, I just find a use for it, wait until the gift-giver is not around, say after a couple days, and just find someway to either not use it or give it to someone that will appreciate it.

It's a bit hackneyed/corny, but I honestly don't think there's such a thing as a "lame" gift. Maybe, not what you hoped for, but not "lame".

I tend to get let down with gifts. The problem with reading a lot is that there are only two types of books people gift you: Ones you've read, and ones you've been avoiding reading for a good reason.

I throw it out the window in front of the people who give me the gift. I mean, who in their right mind would give someone a can opener as a souvenir?

Say thank you, and keep it in the closet/garage. I've even re-gifted some things to other people who do want something of the sort.
