Texas.

Lots of iconic imagery. Wacky survivalist militia groups. Ties to NASA / space program. Mutant cattle. Mutants in cowboy hats.

Brotherhood of Steel - not canon. Could do much more with the place.

I personally think a Fallout game spanning more than one continent would not make much sense. The Enclave might be able to travel the distance, but I think they'd be too patriotic for that (and probably destroyed by the end of Broken Steel anyway. Didn't finish it yet, though), but just thinking about the riverboat to Point Lookout makes travel between continents very unlikely.

Europe would be nice though. The countries around here are not quite as empty, people are more scattered, less high-density areas like DC.

The location should make sense though. I'm not sure if there's a European counterpart to Vault-Tec in the Fallout universe, and that's actually where the problems start.
For Fallout to happen anywhere outside the US, they'd probably need a whole new canon for that area. New companies, New mutations, new factions, very different environment (architecture, roads and landscape) and they'd be almost completely unable to incorporate any lore from the previous games.

Well first of all I'd like to remain in the states so far we've been to the west and we've been to the east... how about we go to the north a little bit (No not Canada) see what happened to Mount Rushmore. =O

Fallout isn't about a nuclear apocalypse, it's about an exaggerated nuclear apocalypse based on the limited scientific knowledge and fear surrounding radiation during the 1950's. Furthermore, it's about how American culture from that time period was affected by this war. Honestly, it makes no sense to make a Fallout game outside of America since it won't be Fallout.

The only place I would even consider outside of America would be Canada. That's only because America had annexed Canada prior to the war, so American culture was already having a huge influence on it. It would be interesting to see how this affected the people living in Canada. Still, it would have to be handled well otherwise it wouldn't be true to the Fallout franchise. Anything else would be missing the point completley.

All of which is based on the culture from the 1950's. The game may take place hundreds of years in the future, but the culture is still from this time period in American history.

There's also the fact that most of the things you commonly associate with the Fallout franchise (Super Mutants, the Master, Poseidon Energy, the Enclave, the Brotherhood of Steel, etc.) could not possibly exist outside of America. How is it a Fallout game if it is missing most of the things that have been staples in the franchise?
The Enclave had Vertibirds and could have easily gotten to a different as long as they had the fuel. Meaning that getting rid of those wouldn't be nessisary.
Poseidon Energy could easily have been a world-wide company. Meaning that getting rid of it wouldn't be nessisary.
I'll agree with the other ones -- though, many FEV viruses [http://fallout.wikia.com/wiki/Forced_Evolutionary_Virus#FEV_Variations] were made in the Fallout series meaning that it could have been made elsewhere, and the Brotherhood may have some Vertibirds capable of crossing lands with the fuel needed.
Also, if people say "No nucluar warheads were dropped on other countries," the UK is allied with the US along with many others, and many are allied with China. That could easily mean a global nucluar threat. No one would know about the other countries being or not being hit, meaning that it is possible that it could have happened.
