First time only Thane died as he was the only one not loyal. All times afterwards no one has died.

when I played good Shepard, everybody survived except yeoman chambers, mainly cause i didn't go into the relay immediately, when I played evil Shep i saved chambers from liquification, but i didnt send anybody with the crew when they went back to the ship so they all died but my squad mates all survived

I lost Garrus and Mordin. Very disappointed in that fact, but I promised myself the my "main" would accept anything that happened so I haven't replayed that character.

The one I am most curious about is that my Insanity playthrough, only Samara and Tali survived. Everybody else was killed, but I still survived the mission. That should make for some interesting ramifications in ME3.

First play through we all made it. I sent Kasumi through the vents and had Garrus escort the crew back.

I'm half way through my second play through so wish me luck.

nobody in my first run. I did lose Jack in my second playthrough - not really sure why that happened. other than that, no other playthroughs have resulted in death - none of my characters are pure paragon/renegade so I feel pretty good about my leadership under fire. also, as others have noted, yes... lots and lots of collectors

On my first playthrough, no one did. Didn't extensively use a guide either, and I haven't really lost anyone in sucessive playthroughs. I am aiming for a playthrough where Tali and Garrus are the sole survivors but I'm not sure if that's possible.

All my crew survived because I always make sure to do every mission and collect every upgrade just on principle.

In my five PS3 playtroughs, no-one died. Not one crewmember, not one squadmate, no-one.

Difficulty:Insanity

Impressive fact: I´m not lying.

No one... I like side-quests.

I did blow up a bunch of bug-people, some zombies, and a real big mutant abomination though.

Nobody dies on my watch.

Even if that watch is my first run through without using a FAQ.

I don't even have the heart to kill off characters I don't like. Although in ME2 that wasn't really a problem since the only character that got on my nerves was Zaeed.

First playthrough got Jacob, Thane, and Miranda killed.
pretty much all my later playthroughs only got Jacob killed, because I don't like him.

I wouldn't worry about it. My characters are never pure paragon or renegade either, but I've always had enough points in one to solve the arguments (and, usually, enough points to choose between either option to do so).

None of my squad-mates have died in any of my many, many playthroughs. Clearly I'm just too good for these games...

I played on normal and no one died. I cared about each and every member of my crew so they were all loyal and fully upgraded.

And I enjoyed planet scanning so that wasn't even difficult to do.

Off-topic: Has anyone noticed that the loading screens told you to choose renegade options to be "the ultimate badass", but being a paragon has some much more intimidating dialogue?

I didn't buy any of the Normandy upgrades so I think everyone possible died on the way there.

I didn't know what the upgrades did...
I'm not sure who actually died once the ship had landed.

My first mission was beyond epic everyone died except garrus and grunt oh and sheperd survived but the feeling when everyone died one by one it really felt like a suicide mission.

On the second play no one died and the ending didn't have the same punch.
