Yukari is the hardest? Is that general consensus or personal opinion (outside of the part where she's the only Phantasm boss)? Regardless, more power to you for getting that far. I keep stockpiling massive amounts of lives in Stage 3 only to waste too many on Youmu.

OP: Same series as the above guy, different game. Touhou 11: Subterranean Animism. Behold the unholy demon that is Orin. Any lives I've stocked up will disappear because of a wheelbarrow holding catgirl in hell. If I manage to beat her, then she comes back in the next stage for bloody revenge.

Opinion and consensus, actually.

That said, I suspect other extra bosses come close on difficulty. No idea which ones, though. Not really sure how to rank them.
General consensus says the Prismrivers for the difficulty spike (and Merlin specifically for curving lasers), Reimu and Marisa for being patterned spam or random spam respectively (and Master Spark), Aya in MoF because of the survival spell, (optionally) Parsee for having homing attacks as a Stage 2 boss, Yuugi for sheer brutality, Satori for copying difficult attacks, Orin for being Orin, Murasa for giant anchors, and Shou for more curving laser goodness. Most of the extra bosses count by default I think. Ranking is probably out of the question, but Orin might take the cake.
Gah, yes. Prismrivers. I still have nightmares. And do not get me started on Aya, or that game in general.

Funny, though. I never had the least bit of trouble with Yuugi, Murasa or Satori. I... kinda thought of those as breather levels, to be honest.

I mostly get blindsided by Yuugi on one specific nonspell and Storm on Mt. Ooe. She's fairly easy otherwise. I can gain a lot of lives in UFO stage 4 but then lose them to Murasa due to some combination of stupid mistakes, getting clipdeath'd by an anchor or stray bullet, and her evil evil survival spell. Satori probably depends on the shot type. I think her Yukari spells have WAY too much health and her Alice patterns just confuse me. I never understood why everyone thinks Shou's curving laser nonspells are hard, but I can never figure out her spinning cross and last spell.

Oh dear, yes. You're right. Storm on Mt. Ooe does usually get me. Also, I'm fairly sure Satori's Suika spells are the worst. They're also incredibly fun, though.

The damned Cerberus in Titan Quest: Immortal Throne. Let me into hell, drat it! STOP KILLING ME IN ONE HIT!

Seriously, I must have fought Cerberus about 30 times (with a two minute trek between fights), but I beat Hades in one go.

I remember the Dawn of war 2 chaos rising boss... a great unclean one i think it was, i must have done something wrong since i had to run in and out of his room to heal my squad since they were dropping like flies. Doing that it took me a good 50 minutes to actually get him down(this was on release day) and im pretty sure he got a nerf in an update later, can't be too sure though.

My hardest gaming experience has got to be storming the Reichstag on COD5 (and the rain level with the Marines) on Insane difficulty

The enclosed enviroment, endless respawns, insane grenade spamming as punishment for going in cover, (I once counted TWELVE grenade symbols landing on me) one-shot near kills from enemies with bolt action rifles, (one hit will put you on the final level of red!) three-hit kills from SMG's and automatics, telescopic vision, the ability of NPC's to see (and shoot) you through walls, and the 'perfect' accuracy enemy NPC's have.

On top of this, friendly NPC's shoot like blind guide dogs and get in your way. They even push you out of cover, and you know they're just laughing at your fragility from being so easy to kill, whilst standing in-front of a heavy MG and shrugging it off!

But the most, THE MOST infuriating this about the game... the pathetic NPC grenade trow animation! The movement is a slight stand-crouch out of cover, and a limp-wrist toss. The result? The grenade goes fifty meters to land right at your feet! Along with all their buddys grenades, with the sole intention to make you stare at that 'watch the grenade indicator' message!

I still have not been able to fully complete this game on Veteran yet, despite sometimes attempting one room for over FOUR HOURS!

The most frustrated I have ever been by a game boss was probably in Dragon Ball Z:Tenkaichi Budokai 2 when I had to last only a one minute or two fighting Cell in his final form with Hercule on hard. It seriously took almost 2 hours for me to actually beat that extermely unfair level, and finally ended with me having put a crack in my controller from hitting it on the floor so much and using all my harnessed rage into a series of extermly cheap attacks that barely got me through.

I now use that crack in the controller to remind me to never get that angry at a game again.

Bartandelus (can't spell) on his first battle, I failed SO HARD it was unreal. It was probably easy for most people but I suck :p P/s I should mention this is Final Fantasy XIII

That I've beaten? Tiamet in Darksiders on Apocalyptic.

That I've tried to beat? Lou from guitar hero 3, hard difficult (I can't even get past cult of personality on expert)

(Oh, and by the way, for all of you saying the Demilich from Baldurs Gate 2 is hard, I've heard the Slayer cannot be Imprisoned)
