Again, I haven't read the books or seen the movies in a long time, so maybe there's an explanation for that and I'm making an ass of myself.
He did raise a few eyebrows...at least, the first time he went there after starting to look like that. That was during his job interview with Dumbledore, as I recall. He was born completely normal. The second and sixth films show him younger. He split his soul into several pieces to make him immortal, and doing so effed him up. Kind of like how your characters always look ugly in RPGs when you're evil.
