The worst ones I know of come from one of the earlier games on the 360.

Solo Champion - Climb to the top of the solo leaderboard
Team Champion - Climb to the top of the team leaderboard
From what I remember there were complaints that it was highly unlikely you could attain all three. It was also highly unlikely most players could attain them at all unless they played the game solid for a week or two.
If you set out to get the team achievement, getting the solo became impossible and vice versa.
Someone really wasn't thinking when they designed these awards.
Achievements to me are something that should be possible to be gained by anyone, not just the people who have no responsibilities that restrict their playing time to one or two hours a day.
