The fist time I got to the Police Station in Resident Evil 3: Nemesis, I ran from Nemesis into the Police Station, so was walking towards the front desk and heard a BANG, I was playing wit a friend, I screamed and told my friend

"OH S&%# OH S&%# HERE HE COMES, I GOTTA RUN"

so I ran towards the next room, after a while the loud bang was heard again

"DAMN HERE HE COMES AGAIN"

so I ran to the next room
I got all the way to the save room
I took my heavy weapons and loaded with ammo and got out determined to KILL Nemesis
guess how I felt when I realized it was just the GOD DAMNED BACKGROUND MUSIC!!!!
also my friend started laughing at me because he had already finished the game and knew that already...

Also in Silent Hill 1, the school, where you find the cat, that damn sound made me jump

EDIT: perhaps all Silent Hills, The radio when there are monsters near by.

The fucking fish in the Windwaker Zelda thing. You throw bait to get the info on an island and you hear 'haaaay' in a strange voice. But considering there's about 50 odd islands, it gets annoying after a while.
