Quick clue - I've recently began a phase of retro-gaming while my 360 was being repaired.
I'm gonna go ahead and guess crash bandicoot. A similar sound haunted most of my childhood.
The most annoying sounds i can think of must be experienced not described.

Red Alert/any RTS; the announcement of any superweapons being built/or preparing to fire, most notably; "chrono-shift detected" when you're playing soviets, and "Deathand APPROACHING" from Dune. Also pretty much any other warning, including, "your base is under attack" and "YOUR DUNGEON HEART IS UNDER ATTACK!!!!"

Wipeout; "Quake"... when you're in first place.... and the finish line is in sight.... and you have barely any shields left...

Resident Evil 3; "STAAARRRRRSSSSSS!!"

Street Fighter; "Hadoken...Hadoken...Hadoken...Hadoken...Hadoken...Hadoken...Hadoken...", etc etc. Also from SF4; THAT song. You know the one I mean.

Warhawk; the sound of a cloaked warhawk while you're on the ground. Usually it's followed by the sound of cluster bombs or a TOW.

Any shooting game; "clickclickclickclick".

Any fighting game; "YOU LOSE!"

Lara Croft's scream when she falls in Angel of Darkness. The controls are so bad you fall off something every five minutes but it would have been more bareable if it wasnt for the screaming. And then the game crashes and the screaming just wont stop...why wont it stop?!

yeah that little riff is really irritating. I guess you could say i does it's job though because it makes you feel as though you've failed in epic fashion :p
Though it fits it is after all Saren theme tune, the rest of the songs not bad actually parts of it are used throuout the game and in trailers....but maybe im just a nerd for having the soundtrack hehe

One sound that always filled me with grief was that one in STALKER, the air sucking sound when you're close to a Controller. Since I play with a variety of mods, this inevitably means a bunch of Controllers supported by an army of zombies. It also becomes ear-bleedingly irritating when the sound gets bugged and stays in one ear for my entire stay in a certain map.

For a sound that ticks me off however it would have to be the noises that the "zombies" in stalker made. They didn't sound like zombies they sounded like someone doing a retard impression.
Well, they basically were retards rather than a traditional zombie, all they had was their brains melted.

*Twang* (Fallout 3 Tripwires)

The sound that the Mirelurks make in Fallout 3. I hate those things, and I always tense up when I hear that low pitched clicking sound they make.

Finally, the agonized shriek from the fast zombies in Half Life 2 when they're mid leap and about to disembowel you. Especially if one of them manages to ambush you from behind.

When a game glitches and plays the same one second sound bit over and over until you kill the program. Twice as bad when the sound is static or some griding sound.

My top 3 are the poison headcrabs in Half Life 2, the chainsaw in Resident Evil 4 (especially when it's faint, because at that point you can't see him yet), and the music speeding up in 2D Mario games.
