In RSV campaign Michael saying map change every time someone throws a smoke, a flash, trips. Also, the beeping noise heard when nearing the end of a street after driving its entire length in Burnout Paradise after playing Halo 3 an entire day; I thought it was someone locking on to me with the missile pod. Lastly, in Fable II if you kill someone and someone saw for the next 2 hours you'll hear, "You are so amazing." "Stay away from me you murderer." Now that I think of it almost all the dialogue from villagers is annoying.

That and the God-awful crunching sound when you fall to far from the first Call of Duty.

For a sound that ticks me off however it would have to be the noises that the "zombies" in stalker made. They didn't sound like zombies they sounded like someone doing a retard impression.

The stuff the NPCs in TES 3: Morrowind say. "What do you want outlander?", "Muthsera", "Good evening outlander", etc. But I kinda got to like it over time :D
Or from Might and Magic 7, when you go to a stable and pick a place to go, you hear "Where do you want to go today?" and then "Our journey begins!". Every. Goddamn. Time. Gets really annoying when you travel from stable to stable.

Poison headcrab in Half-Life 2. I have emptied entire clips into the things. The noise they make terrifies me shitless.

Hmm... a few. I'm thinking sounds all players know as universily bad:
The delicate clinking sound of a gernade landing square between your feet.
The dead man's click (gun out of ammo)
The console powering off for one reason or another
Squeker who forgot their ritalin joining the server
The music of a Tank charging towards you as to disassemble you and your 3 friends.
"I AM SHODAN!"
The sound of a chainsaw revving up right behind you (RE4/5, or GoW... or any game for that matter)

Oh! Forgot about the "NREEEEEEEEE" sound when someone's targetting your tank/airplane/APC.
Anybody remember where "Tactical launch detected" is from? I think Supreme Commander or something.

Any line repeated more then three times will make me get a bit ticked off but the biggest annoyance is when 11 year olds spout profanity in my face no matter what happens.

I'd be fine if they could just grow up and realize cussing stopped being cool after the first day you used it

OK, 4srs this time. Scariest: probably the sound of a turret coming on in Bioshock. For some reason, even though I know the best thing to do is rush it so I can freeze the thing before it starts firing, it's always like a reflex to just dive back wherever I just was. I don't know why I can't get over that, because they're not hard to deal with.

Runner up would have to be the helicopter sound in the Half-Life series that means you just got ambushed by a pack of Antlions, aka the one and only enemy that is ALWAYS programmed to infinitely spawn in those games.

For annoying, I want to say anytime in Bioshock that a lot of stuff is going on at once. I dunno if it's my computer or what, but I seem to be limited in how many sounds can be playing at once, and its approach is to just temporarily mute some of them to make space for others. This includes the NPC monologues. Really breaks immersion. Which sucks because immersion is really key to enjoying that game, more than maybe any other.
