The Click of Death.

Otherwise known as your gun running out of ammo in the middle of a firefight. Especially horrible on the Source engine mod Eternal Silence, given that the guns all seem to take at least a second to reload...

Girl grunts when playing a female character [which is my preference in most games, when given a choice]. I suppose it's a tradeoff..
The vending machines in Bioshock also got a bit old after a few hours.

Does anyone else hate the voice that says "Resident Evil" on the title screen. It was in 4, not sure about 5.

It just makes me think of some guy sitting in a recording studio thinking,
"Okay, what voice would sound really scary?"

an incoming mssg in supreme commander because you just 'know' thats is going to be the guy from high command whinging at you for the umpteenth time to complete the objectives. Also, not nice: "strategic missile launch detected" when you already built a missile defense but GAAAAH it's not ARMED yet!!

Quick clue - I've recently began a phase of retro-gaming while my 360 was being repaired.
I'm gonna go ahead and guess crash bandicoot. A similar sound haunted most of my childhood.
The most annoying sounds i can think of must be experienced not described.

Hmm sometimes when I play TF2, the sound of an incoming chat message is two times as loud as the ingame sound (perhaps because I decreased the ingame sound, but regardless), it always scares the shit out of me the first time, and annoys me terribly after that.

I'd say the great title screen of Japanese games (mainly Capcom and Namco) where some grunty-voiced guy will say: 'Time Crisis...2', 'Resident Evil...4', 'Soul Calibur...2' and 'Devil May Cry 3'. I just shake my head and think, "Yeah...I know...I put the disc in."

Also in Silent Hill 2, along with the previously mentionned static, would be the point where you entered a huge dark hall. The only thing in it is a gallows right in the middle but every-so-often you hear what sounds like a bull charging right at you, but there's nothing there!
