Absolutely not. I'd remember things best left forgotten, there would literally be years of my life where I would be a complete invalid until I developed the fine motor control necessary to take care of myself, and it would be YEARS before "grown-ups" would take me seriously for the things that I said and not just dismiss me because I haven't gone through puberty yet.
Besides, I'm well on the way with this life, why the hell would I quit to restart now? Did I miss an achievement or something?

With all the same knowledge I have now? Hell yes, I would. There's a lot I would do in just the same way, of course, but there are a couple of things I'd do very, very differently... like most of high school, plus starting earlier on building the skills I'm now building for the career I want. And being a better person, too (or at least pretending).
It would be odd, though, navigating through the currents of life to find my girlfriend... again. Who would know nothing about me, although I would know everything about her. Huh - that one might be tough.

I don't know. Probably. I don't particularly have a lot of regrets but being 21 years smarter than everyone else throughout childhood would certainly give me a huge edge. Plus I would have more time to learn and develop skills (for example at this point in time I would have collectively 18-20 years of guitar under my belt as opposed to 6)
Here's the thing: If you go back with the knowledge you know right now, then fix some problems i your life, chances are more problems will appear because of the fixed problems. Yes, it's complicated, but time travel is some complicated stuff.

Yes. I think when you get older (I am 36) you start to reflect more on what might have been. If you had asked me this same question at 20...I would have said no...that I am who I am and changing the past would be unfair/immoral. I used to take the Temporal Prime Directive to heart, but after bearing the scars of my bad decisions and traumas...the TPD can *expletive deleted*
