I've been briefly banned from an IRC channel once, for complaining about an overzealous op while he was in the process of quitting the job because he was so overzealous his personal friend the admin couldn't keep him. Oh and I got banned from Last Days of Fox-Hound's forums for calling the mod an overzealous nitwit more interested in keeping his arbitrary rules than keeping the forum alive, and committer of intercourse with she-donkeys. Really, the guy was nuts. The first page of a board covered in topics months and years old and you're not allowed to post if they're more than a week old. But you may notice a pattern there.
I was banned from almost all Tribe servers for having an offensive name, and in CS I'm banned from most server right off the start because our clan tag is >KkK. which stands for Krazy klown Killaz. Even with a clan website and such for proof we still get banned. It's funny, because the server splits into two groups. Those who are for me being banned because of it, then those who are like,"C'mon it's just a joke!" So, if you see something with >KkK. in a CS 1.6 server, it's more then likely me. 
I've been banned from Youtube and 4chan for acting "too stupid."
All joking aside, I've only ever been banned from TF2 servers. One of them was for asking a pyro for help guarding my sentry while I set it up, and then the pyro started yelling at me. Turned out it was an admin who thought that engineers "aren't team players and don't need help setting up sentries, they are overpowered anyway" (quoting him as closely as possible). I disagreed with him, he threatened to ban me permanently because, again as closely as possible quoting, "I hate whiner noobs like you, I have over 200 hours on this game (note: He only had 4 as an engineer), and I know more than you about all the classes. Stop whining or I ban you." I said he probably didn't play engy much. Ban.
Another time I got banned on a nearly empty server because me and my friend were playing and I was too good. My friend was boosting the heavy acheivements with another heavy, and I was guarding the intel as an engy. After successfully getting a total of 20 kills on the admin while he was a spy, and a further 15 when the two players on the other team both went spy, he banned me and my friend.

banned from xbox live for a day for "God knows why" and from several css for various reasons which would include NOT being american or tking frenches....oohh¡¡ c'mon it's so hard to NOT shoot french people on servers¡¡¡¡

Banned from CSS Servers now and then for owning noobs and them thinking I cheat, and off what was once my favourite site. This was because I had surpassed mostly of the stunters on it (GTAStunting) for suspected cheating in my stunt videos (which I never did either), that site has since become an asshole of a community, which is a shame since it's the Number one GTA stunting portal.
I decided I was too old for Runescape and decided to just screw with everyone. I tried giving a sex education class to people in Varock. Decided to try to hit on any girl player I could find. Go into to the wild and get killed by someone only to put them on my fiends list and drive them insane for killing me. I also tried to report my self for abusing the reporting option. I all so made a list of all thing you cant say on runescape. Apparently CRAP is very naughty word and should never be said by anyone. I got permanently banned in a couple of hours. That was a good day.
Never had a ban or probie on the Escapist, but I've had a few in TF2, the first 3 were for going into rocket tennis games on 2Fort and killing the other team. Twice for being really uber lucky with the sniper against a lot of newbies... Dominating a whole team for 4 rounds isn't the best way to make friends. Funnily enough thats the only time I've actually not sucked at sniper in TF2.
Finally I got banned from 13 servers in one night for being awesome in Mechwarrior 4: Vengeance. I'd developed a variant of the Daishi Assault-class mech, it carried about 8 railguns set to fire simultaneusly, if I get a body hit its a KO. A lot of people don't like someone who can snipe them from further than the draw-distance...
