Bit late to comment, but with the release of Fallout 3 and all, here goes...

Worst Ending: Fallout.
"Hey, thanks for the help. Now fuck off."
Then, with force attacking or Bloody Mess trait, the true ending:
[Overseer turns to walk back inside. Player draws gun and shoots him literally to pieces. The sad remains vainly tries to crawl to the safety of the vault...only to be executed by the player, who then walks away - leaving the vault-dwellers without a leader, thereby forcing them to become independent beings or perish. And probably forcing them to leave the vault as well, as only the Overseer seems to know how it is run (and how to install the bloody water chip

. Just desserts all around. Cue "Maybe" by The Ink Spots. The End.]
I loved that ending all those years ago. Sure, it's depressing and mean, but it is also poignant in a way few games achieve.
Also, the manual for Fallout 2 has the hero from the first game telling the backstory, revealing that he/she helped founding a desert community, led a good life and is now a village elder. See? Happy end after all!
