When you see game HUD items in the corner of your eyes when not playing.
That happened to me when I was heavily playing Mount&Blade (the damage/kill/experience tracker to be percise), I stopped playing it for quite some time soon after.

When you have twice as much video game music in your iTunes library as you do all the other genres combined.

You're quietly convinced you'll do great come the zombie apocalypse/nuclear holocaust/alien invasion, if you can just get your mits on a suit of power armour and a rail gun

When you've just spent an entire weekend playing Oblivion and have become so used to the save function of the game that when you enter the real world again you immediatley contemplate running around like a moron breaking things and assaulting people under the impression that you can reload your last save, then realising with dissapointment that of course any action in real life is a permenant irreversibal act and in your haze of disapointment you go back into your house and continue to try to reach level thirty three.

You have a serious tendency to be conservative with ammo for your uber weapons, even when you run into situations where life is hell if you don't use them, because you might need them later for something really, super hard. Like, some giant enemy crab monster that you can't beat with Shotty and Pist. Then you beat the whole game without hardly ever using them but tripping over x20 more ammo for them than rounds used....

Or instinctively trying to reload every time you use something, regardless of what it is or how many you have left....

Or wondering why the characters you're playing aren't being healed to full in under a second after every fight and why you're not always above 99.85% mana/MP, even when chain casting. Until you remember you let your WoW card run out for a while, to do you some good, and aren't playing your Shaman right now...

When focusing intently on someone you don't want to lose sight of, for whatever reason (we'll assume non-criminal), you can almost see targeting imagery and the scan bar filling up across the bottom as you try to remember where you know them from or etc.

Had been playing Oblivion for at least two weeks straight, then went on a car ride and thought about how easy it would be to just walk in and take everything worth more then 100Gs but i slowly realized that i wouldn't be able to find a thieve's guild member >>;; really kinda woke me up at that point

Its an oldy, but it still applies:

When you hear the achievement/trophy unlocked sounds on random moments in real life.

I get that all the time, it's paranoia inducing.
