GTA: San Andreas, Mission given by Zero using a model plane. Bah.
Mission where you have to shoot down the model planes. BAH!

Killzone 2 when you have to kill Radek or whatever his name was. The end-boss basically. He can turn invisible and one-hit-kill you plus your team mate is a fuckwit.

Also Saren on my first playthrough of Mass Effect.

A non standard difficult game moment for me today when I played co-op through the first two chapters of Lost planet 2 today (having neither of us played it before). It was not difficult because the game was hard (although it was a bit) but I was finding it difficult to shake of the feeling that I may be developing a mutually trusting and respecting opinion of another human being as well as actually quite enjoying his company. And for me that is bad, because I pride myself on my lack of standard or clichéd emotional structure. For example growing fond of another human being is irregular.

Also Sephiroth from kingom hearts.

And Night Terror from Soal Calibur 3

The second mission of the Kurasawa system in Wing Commander 1. The one where you have to kill four heavy fighters in like 10 seconds or they'll take out the defecting destroyer that's your mission objective. Nigh on impossible without cheating.

The mission where you have to fend for yourself with a sniper rifle in Chernobyl while an injured teammate covers you with sniper fire in CoD 4.

Persona 4: Having to fight Rise's shadow (which was a pain in the ass) and then IMMEDIATELY having to fight Teddie's shadow with no save point or chance to heal afterward. That was the second hardest fight in that game and if it wasn't for the fact that the boss had a predictable pattern, I would have.. just.. I don't even know.

Oh, and then there's Namatame's shadow.
Don't.. get me started on that thing.. XD

Seth from Street Fighter 4 annoyed me immensely.
However the hardest moment in a game for me was Call of Duty: World at War's campaign on veteran. So...many...grenades!

Misadventures of Tron Bonne, upgrading a Servbot's speed from 3 to 4, the final challenge. Serving lunch to 38 servbots in 36 seconds, and even a single mistake costing an entire second.

That took away some sanity that will never return...

Makes you realize just how special Servbot 27 is though...

The final section in Killzone 2 before fighting Radec. It's ridiculous. I wasn't even on the hardest difficulty and it was nigh impossible. Oh, didn't manage to pull a headshot on everyone fast enough? Bummer for you, shock troopers have stormed the stairs, have fun getting knifed. And then Rico would constantly die and complain for you to help him up. Then when you think you're doing well rockets fly from the other side of the room and you get insta-killed. God how I hate that level.

Also, as I've said countless times before, the final boss fight on Uncharted 2 on crushing. That almost made me snap the controller in frustration.

CoD:WaW on Veteran difficulty. I just can't do it. I don't have the patience.
Metropolis Zone on Sonic 2. All these years later, it still beats me.
All the stealth side missions on inFamous. I had to retry them so many times. Actually, extend that for most stealth sections, so pretty much all of Splinter Cell: Chaos Theory.

Old King Allant in demon's souls on NG+ which is 40% harder than the first playthrough while trying to avoid being soulsucked at the same time, but it was worth it for the Northern Regalia *drools*.

The FUCKING HALLWAY BIT in "No fighting in the War Room", playing on Veteran difficulty. Modern Warfare was, on the whole, a pretty easy game, but that ONE FUCKING SECTION of that ONE FUCKING MISSION took me THREE FUCKING HOURS to FINALLY FUCKING FINISH.

I mean, the FUCKING MILE HIGH CLUB only took me six tries to finish, so I have no idea why that part was such a pain.

FUCK.

Anything that has PUZZLES!
Don't like it when a game makes you think?

my hardest gaming moment.. umm... that part in MW2 where you gotta run out of the house down the hill to the helicopter, after dieing like 100 times i get to the bottom and get killed.. WTF hard and bullshit.
No. I don't like puzzles in an action game. If I wanted puzzles I would buy World Of Goo.
My apologies, maybe be a little more specific because you said
Anything that has PUZZLES!
so i presumed you didn't like any puzzles, like you said.

Demon's Souls NG+ is pretty ridiculous. Basically your health bar doesn't matter. Go to 3-1, get one-shotted by the wardens's soul ray and paralysis-soul suck attacks, get 1 shot by black phantom arrows, get 1 shotted by the corpse-thing's spell, leave, go to 4-1, get 1 shotted by the skeletons if you are low on stamina, get 1-shotted by the red skeletons, get 1-shotted by Adjudicator's tongue... Fuck you.
