That purple homing magic attack done by the Four Kings in Dark Souls. Yeah, it moves kinda slow and is kinda easy to avoid if you run like a ***** for 15 seconds, but he'll probably send out another one while you're running away. And then another king will show up. And then you're as good as dead. And the lack of depth perception in the abyss just makes it worse. Oh God, I can't even tell how close the damn thing is.
Pro tip man, equip a 2h weapon and havels armor, and just wail on them. The idea is to kill one before the next one spawns. Don't bother being tricky, just kill them.

Most of the boss fights in dark souls can either be exceedingly difficult or relatively simple, if you follow the right strategy. I know everyone says that you can beat all bosses without being hit, but that doesn't mean that it's the best strategy.

Oh, if there wasn't a zombie effect like that that made me unexpectedly lose the game...I wouldn't be complaining about it. Also, I used X-Zone on that Wrex Soul guy. I don't like bullshitty battles.

It's something you have to keep at, because a successful use of it is an instant win when you do. Oh, and I found a vid for the zombie thing!

You see what happened to the one guy towards the end of the vid? Zombie, an actual zombie-type zombie, and I didn't know that {A} the game ever did that or {B} that unexpectedly having your whole crew end up that was was a game over.

Regeneration. I loathe almost killing a boss, then stopping so I can heal a bit, only to have him heal COMPLETELY.
This. Also any pokemon move that lowers accuracy or raises evasion. It drives me insane when I can't hit some trainers Pidgey that's twenty levels lower then me, but keeps using Sand Attack!!

Any enemy attack that can either stagger me or knock me down, with double frustration points if that enemy is able to spam that attack on me or if I'm surrounded by a bunch of the same enemy doing the same attack. As much as I love Saints Row The Third, I have to admit that the brutes and zombies from that game are prime examples.

Laser. I hate that attack. With a burning passion. You need pinpoint precision when you dodge that one hit KO sword beam from hell.

Maya, a boss in Shin Megami Tensai strange journey has a skill that can INSTANT KILL spellcasters.
To be precise, she is weak to no damage type besides gun damage.

Hitting her with gun damage or any element at all causes her to retaliate with an unblockable instant kill.

Then there's a move from one of the potential final bosses in the same game: MA. No element, no way to avoid it, hits one random party member and deals damage equal to their current HP. The boss is healed by this much.

Oh, also, this is a game where you lose if the protagonist dies. And if MA happens to target him? Too bad.

Possibly the final boss from Final Fantasy IX:

Necron's Grand Cross Attack. This attack is able to cast multiple random status ailments on all your party members. INCLUDING ones you can't protect yourself from. Such as Zombie, Berserk, Mini, and DEATH. That attack caused me to retry that battle at least 4 times before I finally got the bastard. Thankfully though on my second playthrough, I was smart enough to bring a bunch of remedies so Quina can use his/her Angel's Snack blue magic whenever Necron used that attack.
I actually avoid that one pretty good. before the battle all the gems on my characters go to all ailment blocking abilities and Auto-regen. Most the stuff not blocked is not so bad and clears up on the inevitable trace given to me from that attack.
I make sure to use abilities with long animations cause that gives regen time to heal me quite well. Plus Hitting my team with a full Curaga makes sure I generally stay well above 60-75% HP.

The overpower attack half the non-human enemies in Dragon Age: Origins had, which was basically their "I win!" move because it was nearly impossible to break and killed you really fast.

The Dawn of War 2 AI has several, including the Ork Kommando Nob's ability to summon a squad of Kommandos... on top of one of your squads, allowing them to activate their Luv tha Dakka ability, which pins your troops to the floor and at point blank, kills them in seconds.

any boss that uses something to recover them selfs.
I HATE THAT!!!
Seriously, that was the worst thing they could do; even worse than spamming Protect or another block move.

Final Fantasy bosses that Heal or Rez themselves for ungodly amounts of health. whn I blast them with raw eldritch power, I expect them to die, not rebirth themselves Phoenix style right when i've exausted my spells/mana.

Dragon Age: Origins - Stinging Swarm.

Nature damage generally isn't a problem in the game...until some Hurlock/Genlock Emissary sends a Stinging Swarm at me. Then I have to chug potions until it goes away.
this, this a thousand times

my advice once you get to the point they do that (usually at the siege of denerim) every time you get in to a fight, pause and look for an emmisary, use ice or crushing prison on him to stun him at the begining

The fat ugly bastard which first poisons, then he activates AOE and then pulls you next to him with AOE still active, I can't remember Heros name or the abilities but if you aren't a Strength hero with maximum HP and 2 levels above him you can't FUCKING SURVIVE!!!
yeah... something like this that you're looking for??
I believe you're talking about:
http://www.playdota.com/heroes/butcher
He's very difficult to control and not very useful if he gets killed early on.

1. His hookshot can be dodged. It is difficult, but possible.
2. His AoE also damages himself if he chooses to use it.
3. Any offensive caster can simply dodge his hook and nuke him to death (at an early level, later on he gains magic resistance).

As for the official topic, I present the effect "silence".
This basically means you cannot use active abilities.
This effect also exists in DotA, in the form of Drow's 6 second silence, or Lucifer's 13 second silence.
If you're a caster, this means you = useless for that duration, no matter what stage you are in the game.
Keep in mind that the longest stun in the game is 6 seconds, an annoyance for all heroes alike.

Sweet Tooth's special attack in Twisted Metal 4 on the PS1. It's almost impossible to dodge, it lifts your vehicle into the air, and does heavy damage over several seconds. What really made it over-powered was that it recharged quickly enough for him to stun-lock you.

The only way I was able to defeat him was by leaving a trail of proximity mines along the roller coaster track.

Healing in a turn based battle, more specifically healing in a one on one turn based battle. I don't know if there are words to properly describe how frustrating it is to be stuck in a cycle of Hit-heal-hit for something like 20 fucking minutes praying for a critical hit or something to inch him past those 2 poxy little HP he keeps holding onto.

The only thing worse is if YOU are the one stuck in the healing position, because YOU can run out of MP but they can't.

Sephiroth In Kingdom Hearts 2... alomst all of his attacks were fucking cheap; Not unblock able/unavoidable, just really fucking annoying in the timing and everything.

Hits you with combo that takes you to 1/10th of your HP. You get away, and while hes slowly walking on the other side of the arena you use Heal. DURING your Healing animation, you can't move/block, and he ALWAYS teleports right in front of me and takes away the last speck of my hp in 1 swipe.

That, and his Sin Heartless Angel move (Which puts you at 1hp and drains all your Mp)
You think that was hard? Try him in the first game. His attack pattern was totally random, and he pulled the same cheap-ass shit with Heartless Angel over and over and over again, not just on his last health bar. In Kingdom Hearts 2, his moves were clearly assigned to how much damage you had dealt, and if you could press triangle when the commands came up, you'd pretty much won. That said, Sephiroth battles are only difficult if you're below level 70. After that, you can destroy him.

If I had to go for a specific power, it'd be Delacroix's rape-lasers from Chaos Legion. Not only do you have to fight 3 bosses one after another without stopping or having much time to heal, you have to dodge nigh-on insta-kill lasers that spread all over the playable arena. It's just insanity. Thanatos is useless.
