A few things:

1. Fixed Camera Angles + Platforming: Seriously, pick one not both for sanity's sake. I rented God of War 3 and in the beginning there was the area where the camera is fixed for no reason so I kept falling into the water because it rotates into a hard position when you need to jump.

2. No saves: Yeah, just play the 1 hour level in one go... OR ELSE!

3. Lack of clarity for saves. I hate when a game says "save" but it doesn't let you start RIGHT where you were in a level because you had to do something. Devil May Cry 4 does this, so why bother have the option to save in the middle of a level if it's not going to work?

4. Making the single player like an afterthought as far as layout, like what Lost Planet 2 did where you're in a virtual lobby. I don't think you should actually be able to confuse SP for co-op or multi.

5. Lack of Common sense for situations. Basically, when you're supposed to ignore the fact that the guard SHOULD see you sneaking across the room so you're stuck until you realize there is some magical path a step to the left that brings about invisibility. This makes forced stealth sections far worse because more often than not the stealth is not a focus of the gameplay to begin with, so it looks even worse.

WEAPON DEGRADATION!

I don't care if YOU call it realistic, weapons don't go from %100 to %5 in a 20-second battle with a goblin who has a shield.

Worst offender: Dead Rising.

One hit melee kills in any game with guns in them. I know that they have to make the melee strong to balance it out, but damn it, I shot you 12 times with my pistol before you stabbed my foot with your knife!

Searching for tiny objects that influence how well you can do in the game later. Yes, I am talking about inFamous and those damn blast shards.

Same except I want to add one more: those fucking music sections in Spirit Tracks. They want us to fucking precision work with the DS mic! That's like trying to paint with a 50 ft. long brush! I start running down the path of immersion and fun, but I have to break down a brick wall using only my face every 100 feet. I swear I must be doing something wrong. That or god hates me.

Well, it's not an instant lose in Arkham Asylum anyway cause you can get away even if you are detected, it's just that you're heavily punished for messing up, which I don't mind.
When you rescue Gordon?
It only happens once or twice, and there its justified I think.

Punishing a player with tedious repetition.

If there is one thing that bugs me (and there isn't much that does), it's forcing the player to replay large portions of a mission/level/whatever due to dying or failing an objective. It's one thing to have to do Mile High Club over and over again (because it's really short) and it's another to have the player traipsing all the way back to their blood stain fighting the same enemies over again in Demons Souls.

COD4, Mile High Club on Veteran difficulty.. complete one of the toughest shooter levels ever made in under one minute.

I can't stand these either... the ones in Metro 2033 and Red Dead Redemption caused me so much rage.

Games where you have one of those are even worse though. They are always insanely expensive or difficult to get, take a long time to cast or use insanely rare ingredients, and the worst of all is that they NEVER work on anyone that it would be usefull to use it on. That super scary, crazy difficult boss is immune to your expensive instant death spell, the former mini bosses that are now regular enemies are immune to it too but fear not those mindless grunts you kill with one hit of your starting weapon has a 50% chance of of getting killed by it with a 100% chance of using up the ingredients either way.

I play 360, and nothing is more annoying than GTA IV's making you hold down A or tap it to move faster than a leisurely stroll, WTF? Since when is WALKING the default movement for video games? walking is for real life, in video games you want to get around fast.

Also, I don't know if this is a problem on the other platforms, but on 360, if you're holding down/tapping A, it occupies your thumb so you can't manipulate the right thumbstick - making looking around while running impossible - is THAT realistic? Did Rockstar expect me to be casually strolling through gunfights? Aaahhh.. so fuckin annoying..
