Pretty much any character you have to escort for a mission (especially the ones who, if killed, force you to restart).
They are pretty much required to do one or more of the following:

1) Charge into a swarm of enemies before you've finished off the current crop.
2) Lose all sense of direction, in spite of their only requirement being to follow you.
3) Get stuck on random objects, like rocks or poles.
4) Walk at a maximum of half your speed.
5) ***** at you for not moving/killing quickly enough.

If you recognize my picture you know exactly who I'm talking about. But these should cover all escortees.

1)"By Azure,by Azure,by Azura! It's the Grand Cham--" That's usually when I bury a Deadric Arrow of Cleansing in his face.
On Dive Rock.
2)"Need a dispensah here!" "Think fast, chucklenuts!" "Bonk!" The scout. While not technicaly an NPC,he ain't controlled by me.
3)Irvine Kinneas. Cocky bastard...Couldn't even shoot the woman who taken care of him for most of his life.

Little Olie from Psychonuats, yeah hearing Ow my Face is funny the first few times, but when it's in the most grating voice known to man.It loses what little humor it has.

Also special mention should go to Yukari from P3 for the idiotic crap she pulls in the Answer. At least the battle music was really cool.

Moria from Fallout 3, she had an annoying voice, her quests were difficult (at the low level you first encountered her) and she didn't even give you any good rewards
I never found them too difficult, and the rewards for the optional objectives were pretty great.

Running into an Enclave trooper on the way back from the store (while dragging around two times your maximum carry weight) was a bit of a problem, though.

The Fanboy from Oblivion.... He just doesn't die.... But he doesn't do anything useful either >.<
Heh, he's good for training up weapon skills for free. Never turn down a free punchbag, especially an annoying free punchbag!

OT: Alistair from DA:O. Just sssoooo annoying and whiny. I don't think I've ever wished more harm on a virtual creation than I did on Alistair.

Glarthir (the paranoid dude in Skingrad) in Oblivion. I just hated his face and how 'enthusiastic' and overacting the voice actor was. I hated him so much in fact I didn't kill him myself or sic the guards on him. Instead I let him kill someone (and then let the guards kill him), so that not only was he dead, but everyone hated him.
Got a funny story there, actually. Went onto my house's balcony, sniped him with an arrow but didn't kill him. He then assumed the conspiracy had gone into high gear, and took out his axe, attacking the nearest person. The guards took care of him rather quickly. :D

I don't know if any of you played the very first fable game. (Back when it was just another fantasy RPG, and funnily enough, included a story) But there was one quest where you had to rescue a little kid from a cave of Hobbes.

My God it was bad, the whole way he absolutely insists on standing right where the enemies are, and has a health bar shorter than a critter's. On top of that, he has without a doubt, THE WORST voice acting ever to be cruelly inflicted on a players ears. His tone of voice his so bloody annoying and constantly whiny that it would make anyone pro-choice.
My wife and I still make fun of that kid.

*whiny, childlike voice* "Ooooh, Oh no!"

*manly, adult voice* "Yay!"

Alternating back and forth the entire time. It was surreal.
